Article Abstract

We introduce the innovative use of technetium-99m-labeled macroaggregated albumin to diagnose high-output heart failure in a patient with multiple myeloma with persistent congestion symptoms. Symptom resolution occurred with lenalidomide and steroids. This marks the first clinical use of technetium-99m-labeled macroaggregated albumin for clarifying high-output heart failure etiology.

Background: Posterior wall isolation (PWI) is often performed in addition to pulmonary vein isolation (PVI) during ablation of persistent atrial fibrillation (PersAF) to decrease recurrence. However, in studies using radiofrequency (RF) energy, the addition of PWI failed to demonstrate improved outcomes compared to PVI-only, possibly due to challenges in achieving durable PWI. An aortocaval fistula caused by rupture of a right common iliac arterial aneurysm is a rare but potentially fatal condition that can progress to acute heart failure and even multi-organ failure due to abnormal shunting of blood through the fistula. We report a case of a patient with such a fistula who was rapidly diagnosed, and following emergent surgical consultation, underwent intra-aortic balloon occlusion prior to laparotomy. This intervention promptly stabilized the patient's circulation by addressing the high-output heart failure.
