We propose that the logic of a genie - an agent that exploits an ambiguous request to intentionally misunderstand a stated goal - underlies a common and consequential phenomenon, well within what is currently called proxy failures. We argue that such intentional misunderstandings are not covered by the current proposed framework for proxy failures, and suggest to expand it.

Background/objective: Plate failure, including bending, is a critical issue in orthopedic fracture fixation, with clinical failure rates of 3.5%-19%, burdening patients and healthcare systems. Preclinical ovine models have observed similar plate bending due to overloading.

Importance: Heart failure with preserved ejection fraction (HFpEF) is a major cause of hospitalization, often occurring in patients with cardiometabolic comorbidities such as obesity and type 2 diabetes. Although early trials of semaglutide and tirzepatide have shown promising results in improving symptoms, those findings were based on few clinical events, leaving treatment recommendations uncertain.
Objective: To evaluate the effectiveness and safety of semaglutide and tirzepatide in patients with cardiometabolic HFpEF in clinical practice.

Respiratory infections are the leading cause of acute decompensation, hospitalization and higher mortality in heart failure (HF) patients. Despite recommendations, vaccination coverage against pneumococcus and influenza remains alarmingly low (3% and 50% in 2018, respectively). This study aimed to identify factors associated with vaccination among HF patients using data from the Nationwide Health Claims Database for 2020, in France.

According to the Region of Proximal Learning framework, curiosity emerges when there is a feeling of almost knowing a piece of information-but what leads to a feeling of almost knowing? Here, we demonstrate that cue familiarity-detection increases curiosity. We used the recognition without cued recall method to experimentally manipulate cue familiarity during recall failure. Consistent with prior research, our manipulation was successful at increasing perceived cue familiarity during recall failure.

Background: How to identify suspected infection for sepsis surveillance purposes remains a well-recognised challenge. This study aimed to operationalise suspected infection for sepsis surveillance by developing an interpretable machine learning (ML) model for retrospective identification of patients with sepsis.
Methods: This multicentre cohort and machine learning study was conducted in two Dutch tertiary care hospitals.
