Article Abstract

(1) : Paraphilic disorders, marked by intense sexual fantasies and behaviors, present formidable challenges. This review addresses concerns fueled by scandals and child abuse. Emphasizing paraphilias' complexity, it systematically reviews the pharmacotherapy literature, aiming to enhance understanding and guide future research. (2) : A comprehensive search from 1990 to 2023 across major databases identified 28 relevant English-language studies. Inclusion criteria focused on adult pharmacotherapy for paraphilias, and results were evaluated using the Newcastle-Ottawa Scale. (3) : Synthesizing data from selected studies, diverse treatments such as SSRIs and antiandrogens were analyzed, revealing variable effectiveness and side effect profiles. Poor quality of the current literature has been reported. (4) : Highlighting the pivotal role of the serotonergic system, this review underscores the efficacy of SSRIs and androgen deprivation therapy. GnRH analog-associated side effects and the importance of a combined assessment approach are discussed. Critical insights contribute to understanding and ethical considerations in paraphilic disorders.

Penile strangulation is a rare urological emergency that can lead to severe complications such as ischemia, necrosis, and urethral injury. We present a case of a 76-year-old man who presented to the emergency department with severe penile pain and swelling after applying five metallic rings to enhance erection during sexual intercourse. The patient had a history of erectile dysfunction and had been using phosphodiesterase inhibitors with a moderate response.

Pedophilic disorder has been linked to neurodevelopmental differences in diagnosed individuals. One common biomarker for atypical neurodevelopment are elevated levels of atypical handedness as individuals with several neurodevelopmental disorders show increased levels of left-, mixed-, or non-right-hand preference. A few primary studies have indicated that atypical handedness could also be more prevalent in pedophilia and child sexual offenders.
