: ISUP grade group discordance between prostate biopsy and radical prostatectomy (RP) impacts treatment decisions in over a third (~25-40%) of prostate cancer (PCa) patients. We aimed to identify ISUP grade migration predictors and assess the impact of preoperative imaging (MRI) in a contemporary Romanian PCa cohort. : We retrospectively analyzed 142 PCa patients undergoing RP following biopsy between January 2021 and December 2024 at Pius Brinzeu County Hospital, Timișoara: 90 without and 52 with preoperative MRI.

Variability in prostate biparametric MRI (bpMRI) interpretation limits diagnostic reliability for prostate cancer (PCa). Artificial intelligence (AI) has potential to reduce this variability and improve diagnostic accuracy. The objective of this study was to evaluate impact of a deep learning AI model on lesion- and patient-level clinically significant PCa (csPCa) and PCa detection rates and interreader agreement in bpMRI interpretations.

Materials And Methods: Clinical data from 503 patients who underwent radical prostatectomy (RP) at Jiangsu Provincial People's Hospital between June 2022 and October 2024 were collected. A stratified random sampling method was used to divide the patients into a training set and a validation set at a 7:3 ratio.
