Acute pyelonephritis is a common renal infection that can become life-threatening when complicated by structural abnormalities, resistant pathogens, or systemic comorbidities such as diabetes mellitus. While common complications include renal abscess and sepsis, spontaneous subcapsular hematoma is a rare but serious manifestation that requires prompt recognition and intervention. This article reports the case of a 54-year-old female with a history of diabetes mellitus, hypertension, and hyperlipidemia, who presented with fever, flank pain, vomiting, and gross hematuria.

This report details the case of a 64-year-old male with well-controlled type 2 diabetes mellitus and hypertension. The patient presented with a 20-day history of progressive dyspnea, cough, and intermittent fever, which worsened despite antibiotic treatment. The initial assessment revealed leukocytosis, neutrophilia, and abnormal chest computed tomography (CT) findings, which led to a provisional diagnosis of pulmonary infection.

Renal cell carcinoma (RCC) is the most frequent form of kidney cancer and represents a small percentage of all adult cancers. Metastasis of RCC to the bladder is exceptionally uncommon, with only a limited number of cases documented, the majority being of the clear cell subtype. Instances involving the chromophobe variant are particularly rare.

Invasive Klebsiella syndrome (IKS) is a significant health concern, particularly in Asia, as it results in a high mortality rate. Due to the hypervirulent strains of , it is more frequently detected among immunosuppressed individuals and will mainly metastasize to different organs, including the liver, kidneys, eyes, and brain. Here, we present the case of a 70-year-old man with underlying diabetes mellitus who developed multiple organ abscesses secondary to , requiring prolonged antibiotic treatment and abscess drainage.

Xanthogranulomatous pyelonephritis (XGP) is a rare, severe form of chronic kidney infection that mimics renal malignancies, often leading to diagnostic challenges. We present the case of a 36-year-old woman with a 2-month history of progressive lower back pain, fever, and weight loss. Imaging revealed left renal enlargement with multiple low-attenuation areas and large staghorn calculi, suggestive of XGP.
