Objective: To examine the strengths and limitations of overlap weighting in observational studies, and to clarify when it is appropriate to use this method based on the target estimand.
Study Design And Setting: This is a narrative commentary that reviews recent methodological developments and real-world examples to highlight how overlap weighting operates, when it provides advantages over methods like IPTW, and the importance of aligning analytic methods with the causal question and estimand.
Results: Overlap weighting produces bounded, stable weights and achieves exact mean covariate balance in the subset of patients with overlapping treatment probabilities near 0.

Introduction: Overlap weighting has emerged as a valuable method for addressing confounding in observational studies, particularly in real-world data settings characterized by imbalanced covariates and limited overlap between treatment groups. Its ability to produce stable, interpretable estimates makes it an attractive alternative to inverse probability of treatment weighting (IPTW), which can suffer from extreme weights and instability.
Areas Covered: This report outlines the methodological basis of overlap weighting and contrasts it with IPTW.

Background: The World Health Organization recommends evaluation of all household contacts (HHC) of index tuberculosis (TB) patients for TB disease (TBD) and TB infection (TBI). Tests to identify TBI and TBD are preferred but can be skipped in persons living with HIV and children <5 years. There is equipoise on the need for these tests in other HHC.

This mini-review critically evaluates the contemporary evidence regarding racemic epinephrine's role in pediatric croup management. As the pharmacological mainstay for acute airway obstruction, racemic epinephrine demonstrates rapid efficacy through dual α₁-adrenergic vasoconstriction and β-mediated bronchodilation, achieving clinically significant Westley Croup Score reductions (2-3 points) within 30 min of administration. Current evidence establishes therapeutic equipoise between racemic and L-epinephrine formulations, though important disparities exist in global accessibility and cost-effectiveness.

There is equipoise regarding the use of closed-incision negative pressure therapy (ciNPT) versus conventional dressings for abdominal incisions in deep inferior epigastric perforator (DIEP) flap breast reconstruction. The primary objective was to determine the feasibility of conducting a randomized controlled trial (RCT) comparing ciNPT versus conventional dressings for abdominal incisions in DIEP flap breast reconstruction. A parallel, between-group randomized controlled pilot trial was conducted at two academic breast reconstruction centers.
