Myeloproliferative neoplasms (MPNs) are the leading causes of unusual site thrombosis, affecting nearly 40% of individuals with conditions like Budd-Chiari syndrome or portal vein thrombosis. Diagnosing MPNs in these cases is challenging because common indicators, such as spleen enlargement and elevated blood cell counts, can be obscured by portal hypertension or bleeding issues. Recent advancements in diagnostic tools have enhanced the accuracy of MPN diagnosis and classification. While bone marrow biopsies remain significant diagnostic criteria, molecular markers now play a pivotal role in both diagnosis and prognosis assessment. Hence, it is essential to initiate the diagnostic process for splanchnic vein thrombosis with a mutation screening, but a comprehensive approach is necessary. A multidisciplinary strategy is vital to accurately determine the specific subtype of MPNs, recommend additional tests, and propose the most effective treatment plan. Establishing specialized care pathways for patients with splanchnic vein thrombosis and underlying MPNs is crucial to tailor management approaches that reduce the risk of hematological outcomes and hepatic complications.

Aims: To examine the association between elevated body mass index (BMI) and a wide range of vascular and cardiometabolic diseases in men and women.
Materials And Methods: This retrospective cohort study used data from the IQVIA Disease Analyzer database, comprising anonymized records from over 3000 office-based physicians in Germany. We included 233 730 patients aged ≥40 years with at least one recorded BMI measurement between January 2005 and December 2023.
Lemierre's syndrome (LS), otherwise known as postanginal sepsis, is a frequently overlooked condition characterized by septic thrombophlebitis of the internal jugular vein (IJV), usually caused by oropharyngeal infection. However, ear space (otogenic) infections are one of the atypical causes of LS and have been rarely reported. We present a case of a male in his 20s with a history of recurrent acute otitis media (RAOM) who presented with purulent ear discharge, fever, neck pain, and swelling for a week.

Pregnancy in women with Ehlers-Danlos syndrome (EDS) carries elevated risks, including prematurity, hemorrhage, and maternal morbidity, posing significant anesthetic challenges. We present the case of a 36-year-old woman with classical EDS (cEDS) and multiple comorbidities, including postural orthostatic tachycardia syndrome, bronchial asthma, congenital adrenal hypoplasia, and chronic venous thrombosis, who underwent an elective cesarean section. A multidisciplinary team developed a comprehensive perioperative plan featuring ultrasound-guided spinal anesthesia, extended post-anesthesia observation, and coordinated recommendations from cardiology, hematology, endocrinology, pulmonology, and other specialties.

Unopposed platelet activation can be associated with pathologic thrombosis. An intact growth arrest-specific gene 6 (GAS6)/Mer receptor tyrosine kinase (MERTK) signaling pathway contributes importantly to potentiating platelet activation triggered by molecular agonists ex vivo and thrombus stabilization in vivo. We describe, herein, the inhibition of platelet function and stable thrombus formation conferred by iMer, a naturally occurring MERTK splice variant, that acts as a GAS6 decoy receptor and decreases phosphorylation of MERTK.
