Warthin's tumor (WT) is a benign and frequent salivary gland tumor primarily affecting the parotid gland. In some cases, this tumor can involve the extra parotid region and affect cervical lymph nodes. Fine-needle aspiration can be the first step in the diagnostic approach to lymphadenopathy; however, specimens from intra-nodal WT can present a potential pitfall, leading to a misdiagnosis of metastasis. Here, we report an unusual case of a patient with bilateral WT in parotid lymph nodes misdiagnosed as metastases. In addition, we highlight the cytopathological aspects of WT to alert cytopathologists about this challenging diagnosis.

BACKGROUND Salivary gland neoplasms present a diagnostic challenge due to their heterogeneous nature and varying histological subtypes. Recent studies have highlighted the potential role of inflammatory biomarkers, such as neutrophil to lymphocyte ratio (NLR) and systemic immune inflammation index (SII), in enhancing diagnostic accuracy for distinguishing between benign and malignant parotid gland tumors. To study the clinicopathological features of malignant transformation of the epithelial component of a Warthin tumour into adenocarcinoma. The histological and immunophenotypic features of the malignant transformation of the epithelial component of a Warthin tumour into adenocarcinoma were studied, and the literature was reviewed. The patient had a history of surgery for a Warthin tumour 3 years previously, and the typical histological manifestations of a Warthin tumour were observed under the microscope following biopsy.

Warthin tumor is uncommonly associated with lymphoma. We describe a case of a 73-year-old man with persistent left neck swelling in the submandibular region. Core needle biopsy with aspiration showed Warthin tumor and small lymphocytic lymphoma (SLL)/chronic lymphocytic leukemia (CLL).

Warthin tumours (WT), the second most common benign salivary gland tumour, are histopathologically composed of bilayered oncocytic epithelial cells with occasional metaplastic epithelium. Tuft cells, which are chemosensory epithelial cells, are present in WT. Tuft cells serve various roles by producing physiologically active substances, such as prostaglandins (PGs).
