Article Abstract

The syndrome of inappropriate ADH-secretion (SIADH) is a common cause of low sodium levels with diverse aetiology. Here, we report a case of a 41 years old male patient diagnosed with SIADH and a good response to Tolvaptan therapy. Of interest, as a potential unique cause, magnetic resonance imaging revealed a micronodular structure in the posterior pituitary, while no other common cause of SIADH could be identified. Hence, to the best of our knowledge, this is the first case of a Tolvaptan-responsive SIADH associated with a pituitary micronodular structure.

Systemic chronic active Epstein-Barr virus disease (sCAEBVD) can primarily involve skeletal muscle to form CAEBV-myopathy (CAEBV-M), which may resemble idiopathic inflammatory myopathies (IIMs). This study reports an 11-patient, 10-year cohort of CAEBV-M to summarize clinicoseropathologic features. CAEBV-M typically affects young adults (median: 29 years), with universal limb swelling, frequent disseminated muscle involvement (73%), and systemic symptoms like fever (82%), splenomegaly (82%), and hemophagocytic lymphohistiocytosis (55%).

A 60-year-old woman with rheumatoid arthritis and an impaired consciousness was referred to our hospital. Prior to transfer, the patient underwent intensified immunosuppressive therapy under a diagnosis of rheumatoid vasculitis that had developed in her buttocks. Variant classification in genetic testing often culminates in "uncertain" calls, known as variants of uncertain significance (VUS), which remain a major clinical challenge. Among the established criteria for variant classification, population allele frequency (AF) is fundamental, yet under-representation of non-European groups hinders accurate interpretation. In this study, we evaluated the impact of population-specific AF on the reclassification of VUS and conflicting variants in hereditary cancer genes.
