Drug-induced delayed hypersensitivity reactions (DHRs) is still a clinical and healthcare burden in every country. Increasing reports of DHRs have caught our attention to explore the genetic relationship, especially life-threatening severe cutaneous adverse drug reactions (SCARs), including acute generalized exanthematous pustulosis (AGEP), drug reactions with eosinophilia and systemic symptoms (DRESS), Stevens-Johnson syndrome (SJS), and toxic epidermal necrolysis (TEN). In recent years, many studies have investigated the immune mechanism and genetic markers of DHRs. Besides, several studies have stated the associations between antibiotics-as well as anti-osteoporotic drugs (AOD)-induced SCARs and specific human leukocyte antigens (HLA) alleles. Strong associations between drugs and HLA alleles such as co-trimoxazole-induced DRESS and (Odds ratio (OR) = 45), dapsone-DRESS and (OR = 122.1), vancomycin-DRESS and (OR = 403), clindamycin-DHRs and (OR = 55.6), and strontium ranelate (SR)-SJS/TEN and (OR = 25.97) are listed. We summarized the immune mechanism of SCARs, update the latest knowledge of pharmacogenomics of antibiotics- and AOD-induced SCARs, and indicate the potential clinical use of these genetic markers for SCARs prevention in this mini review article.

Aims: Sarcoid myopathy (SaM) is characterised by granulomatous myositis (GM) and can overlap with inclusion body myositis (IBM), a late-onset chronic idiopathic inflammatory myopathy with a still enigmatic pathogenesis. As GM can occur in different clinical contexts, we aimed to examine the histomorphologic features and gene expression profiles in cases of definite SaM that may inform diagnostic and therapeutic considerations.
Methods: We performed a multidimensional characterisation of muscle biopsy specimens from patients with 'pure SaM' (n=17), SaM with concomitant IBM (SaM-IBM) (n=2), including histopathologic and ultrastructural analysis in addition to quantitative real-time polymerase chain reaction.

In wheat allergy dependent on augmentation factors (WALDA), allergic reactions occur when wheat ingestion is combined with exercise or rarely other augmentation factors. We analyzed clinical characteristics and disease burden in recreationally active and trained individuals with WALDA diagnosed by oral challenge test. Inhaled and intranasal corticosteroids are widely used in the management of allergic respiratory diseases. Delayed drug hypersensitivity reactions to budesonide are a rare adverse drug reaction characterized by non-immunoglobulin E (IgE)-mediated clinical manifestations, including localized or systemic contact dermatitis, mucosal edema, and paradoxical worsening of pre-existing symptoms. However, such reactions are often underdiagnosed due to atypical presentations.

Dermal fillers have gained increasing popularity for their ability to enhance facial symmetry, restore volume, and improve skin texture. However, their use in patients with cancer undergoing active chemotherapy and radiation therapy poses unique challenges, as these treatments can alter both the safety profile and efficacy of filler procedures. Chemotherapy can interfere with normal wound healing and immune responses, warranting a more cautious and individualized approach when considering dermal fillers in this population.
