Background: Pacemaker implantation has become a routine procedure in contemporary cardiology. Several possible complications during and after the procedure have been described, with this article focusing on the rare complication and the prevention of left-sided lead placement after arterial puncture.
Case Summary: A 90-year-old female patient was admitted to our hospital due to recurrent transient ischaemic attacks following a dual-chamber pacemaker implantation six weeks earlier.

We present the case of an 88-year-old female who presented to the cardiologist's clinic with symptoms of palpitations, dizziness, and fatigue. After cardiac monitoring, the patient was diagnosed with symptomatic bradycardia and required a pacemaker. During the pacemaker implantation procedure, attempts to access the subclavian vein were unsuccessful, and venography revealed severe venous spasm.

Metastatic melanomas with prolonged survival are becoming increasingly common. We present the case of a 47-year-old man with melanoma who developed asymptomatic cardiac metastases, whose prognosis depended on their response to either systemic or focal treatment. Consequently, a CyberKnife radiotherapy treatment was performed using a pacemaker for tracking.
