Patient engagement (PE) has evolved from an emerging concept to a fundamental ethos underpinning healthcare research and communication. In this commentary, we explore the historical evolution in medical research from patients being participants in clinical trials to becoming integral partners in communicating medical research findings. The progression from "why" to "how" PE should occur represents a fundamental shift in the medical publication landscape.

Acute ischemic stroke (AIS) remains a leading cause of mortality and long-term disability globally, with survivors at high risk of recurrent stroke, cardiovascular events, and post-stroke dementia. Statins, while widely used for their lipid-lowering effects, also possess pleiotropic properties, including anti-inflammatory, endothelial-stabilizing, and neuroprotective actions, which may offer added benefit in AIS management. This article synthesizes emerging evidence on statins' dual mechanisms of action and evaluates their role in reducing recurrence, improving survival, and mitigating cognitive decline.

Purpose Of Review: Patients living with cancer are at risk for significant potential cardiovascular complications as a direct result of cancer treatment or due to underlying comorbid cardiovascular disease. This article reviews the methods of risk stratification as well as pharmacologic and nonpharmacologic approaches to cardioprotection in cardio-oncology.
Recent Findings: Several cancer-specific risk stratification tools have incorporated variables such as age, sex, cancer subtype, traditional cardiovascular risk factors and cancer treatment-related parameters to assess cardiovascular specific risk prior to cancer therapy.

Myocardial fibrosis, a key pathological feature of hypertensive heart disease (HHD), remains diagnostically challenging due to limited clinical tools. In this study, a FAPI-targeted uptake mechanism previously reported by our group, originally developed for tumor imaging, is extended to the detection of myocardial fibrosis in HHD using [F]F-NOTA-FAPI-MB. The diagnostic performance of this tracer is compared with those of [F]F-FDG, [F]F-FAPI-42, and [F]F-NOTA-FAP2286, and its potential for fluorescence imaging is also evaluated.

Background: Invasive coronary physiology including fractional flow reserve (FFR), instantaneous wave-free ratio (iFR), and coronary flow reserve (CFR) are guideline-endorsed tools to guide the management of coronary artery disease (CAD). Complex factors impact and confound these assessments, and discordance between modalities complicates clinical management. iEquate is a prospective observational trial that combines multi-modality coronary physiology and optical coherence tomography (OCT) to identify the determinants of pressure-wire derived myocardial ischemia and iFR-FFR discordance.
