Article Abstract

Despite the many accomplishments since the introduction of percutaneous dilational (PDT) tracheostomy, we continue to observe emerging complications as we forge ahead in intensive care practice. As a result of this, we provide a new technique that could avoid some complications, notably posterior tracheal wall injury, bronchoscopic or endotracheal tube puncture, and false track. The new technology was evaluated on a 75-year-old Caucasian male cadaver was selected for a novel PDT technique. A wire with a sharp terminal end passed through the bronchoscopic channel and punctured the trachea from inside towards the skin. The wire was pulled and directed towards the mediastinum. The rest of the technique was carried out like a routine procedure. The procedure was technically feasible; further clinical trials are required to confirm the validity of this technique.

Background: Percutaneous nephrolithotomy (PNL) is a gold-standard procedure for managing complex kidney stones. It is traditionally performed in the prone position. Supine PNL offers benefits, such as enhanced ergonomics and simultaneous retrograde surgery.

Purpose: Pelvic ring fractures involving the iliopubic rami can cause functional impairment. Percutaneous retrograde fixation is a less invasive procedure when compared to traditional open approaches, however precise anatomical knowledge is crucial for safe screw placement. This study aims to describe the morphology of the iliopubic rami, define a safety corridor for percutaneous screw fixation, specially focusing on the relationships between the iliopubic rami and neurovascular structures.

Percutaneous nephrostomy catheter fragmentation is an uncommon complication that is managed through different approaches. In this report we describe an iatrogenicly fragmented nephrostomy catheter in a patient with an ileal conduit, that was removed by using combined cystoscope and fluoroscopy guidance through a retrograde trans conduit approach.

Percutaneous pelvic screwing (PPS) enables fixation of traumatic or atraumatic fractures with little or no displacement, or displaced but reduced fractures, and preventive fixation of primary or secondary tumoral lesions. It is a relatively recent technique, and indications are evolving with progress in pre- and intra-operative imaging. Morbidity is lower than with open surgery.

Case Summary: We present a case of a 31-year-old man with a history of aortic valve endocarditis and surgical aortic intervention. Computed tomography revealed a complex serpiginous fistula with 2 openings between the left ventricular outflow tract and the left atrium. Using 3-dimensional printing simulation for device fit testing and planning, the "mother-in-daughter" system, multimodality guidance with computed tomography angiography C-arm prediction, and 3-dimensional transesophageal echocardiogram guidance successfully guided an 18-mm Cribriform Amplatzer device deployed via a retrograde approach.
