Article Abstract
Policy Points A decade after failing to make it into the Affordable Care Act, the public option reemerged as a health reform goal at both the national and state levels, with polls reporting strong, bipartisan support. A 2020 poll that probed both support for one public option approach (Medicare "buy-in") and attitudes toward government suggests that differences in these attitudes could plague reform advocates' efforts. Although the COVID-19 pandemic viscerally highlighted the need for a more coherent health care system-including universal coverage-other recent evolutions in the broader US political context could undermine reform.

Background: Chagas heart disease (ChD) is a significant public health concern in Latin America, contributing to a high incidence of sudden cardiac death (SCD). Despite advances in heart failure treatment, management of Chagas cardiomyopathy has not progressed accordingly. While ICDs are effective for primary and secondary prevention in other conditions, patients with ChD often experience more frequent episodes of ventricular tachycardia, and ICD use may provide a negative impact and increase mortality.

Triple-negative breast cancer (TNBC) lacks expression of estrogen receptor (ER), progesterone receptor (PR), and HER2, and remains one of the most aggressive and therapeutically challenging breast cancer subtypes, marked by early relapse, metastasis, and limited targeted treatment options. In a recent study published in The Journal of Pathology, Kuo et al provide compelling evidence that nicotine exposure, whether from tobacco smoke or e-cigarette vapor, drives TNBC progression by promoting stem-like and metastatic phenotypes. Integrating clinical datasets, patient tissues, cell lines, and in vivo models, the authors demonstrate that nicotine enhances tumor aggressiveness via coordinated upregulation of CHRNA9 and IGF1R.

Background: The rising rates of cesarean sections (CS) globally have increased the number of women with prior cesarean deliveries who may be candidates for vaginal birth after cesarean (VBAC). This study explores the factors influencing the success of VBAC in women with a previous CS and fetomaternal complications associated with VBAC failure.
Methods: This cross-sectional study was conducted at the obstetrics unit of Ayub Teaching Hospital, Abbottabad, Pakistan, enrolling 102 eligible women who had previously undergone a CS and were offered the option of attempting a vaginal delivery.
