Background: Extra axial abscess of the brain is a rare entity, moreover, extra-axial abscess concomitant with intraparenchymal purulent collections are scarcely reported in the literature. Etiology includes penetrating trauma, paranasal sinusitis, mastoiditis, craniospinal surgeries, and the rare spread of infectious agents through the hematogenous route.
Case Description: We present a case of a young male with Burkholderia pseudomallei Central Nervous System (CNS) melioidosis, forming abscesses in extra-axial and intraparenchymal planes without contiguity.
Conclusion: This is to emphasize the importance of MR spectroscopy and other convenient methods in differentiating the etiology in cranial infections.

Objective: To analyze the changes over the last 2 decades in clinical presentation, initial management, bacterial etiology, and rates of complications of acute mastoiditis (AM) in children.
Study Design: We conducted a retrospective study including all children diagnosed with AM between 2021 and 2024 in a French tertiary care center. Age at diagnosis, first-line surgical and medical treatment modalities, complications and bacteriological findings were collected.

Baastrup's disease is a degenerative condition characterized by the close approximation of adjacent spinous processes. Interspinous bursal cysts may extend into the epidural space, resulting in symptomatic central canal stenosis. Lumbar multiple spondylolysis, characterized by defects in more than one lumbar lamina, is a rare condition often associated with trauma or repetitive stress.

Tegmen defects associated with cerebrospinal fluid (CSF) leaks are a rare pathology that can result in severe complications if left untreated. There is no universal optimal surgical algorithm for repair, although the most common techniques are the middle fossa craniotomy (traditionally 25 cm in area), the transmastoid approach, or both. Here, we describe successful use of a keyhole mini-craniotomy, only 6 cm in area, without mastoidectomy or days of lumbar drainage.

Background: Bibrachial amyotrophy associated with an extradural CSF collection and infratentorial superficial siderosis (SS) are rare conditions that may occasionally mimic ALS. Both disorders are assumed to be due to dural tears.

This study was a retrospective study conducted from October 2020 to October 2022 on 106 posttraumatic patients with acute extradural hematomas (EDHs) who were initially planned for conservative treatment. 74 patients had spontaneous EDH regression (EDHR), while 32 patients developed EDH progression (EDHP) and were shifted for surgery. The two groups were statistically compared regarding the different demographic, clinical, and radiographic factors to identify the significant predictors for regression versus progression of acute posttraumatic EDH.
