This editorial examined the evolving landscape of transcatheter aortic valve replacement (TAVR) as an alternative therapeutic strategy for AR, particularly in patients deemed high risk for surgery. We explored the anatomical and pathophysiological disparities between AR and aortic stenosis (AS) and elucidates the technical nuances of TAVR procedures in AR patients, emphasizing the need for precise prosthesis positioning and considerations for excessive stroke volume.

Background: Leaflet thrombosis and transcatheter heart valve dysfunction are key concerns following transcatheter aortic valve replacement (TAVR). Prolonged neo-sinus washout time (NWT) may predispose patients to hypoattenuated leaflet thickening (HALT) and leaflet thrombosis, increasing the risk of valve degeneration. This study evaluates the association between in vivo NWT derived from aortograms using computer vision and hemodynamic outcomes at 30 days and 1 year post-TAVR.

Background: The computed tomography selection of patients with bicuspid aortic stenosis for treatment with balloon-expandable valve (BEV) transcatheter aortic valve replacement (TAVR) is uncertain. We therefore evaluated a novel sizing algorithm for SAPIEN 3 BEV.

Background: Severe calcific mitral stenosis is common and therapeutically challenging. Intravascular lithotripsy (IVL) can facilitate percutaneous balloon mitral valvuloplasty in patients not amenable to conventional therapies. We describe a modified technique using larger IVL balloons to ensure maximal annular contact and delivery of ultrasonic shockwaves to restore mitral leaflet pliability and reduce transvalvular gradients without the need for noncompliant valvuloplasty balloons.
