The European Society of Gastrointestinal Endoscopy and United European Gastroenterology have defined performance measures for upper and lower gastrointestinal, pancreaticobiliary, and small-bowel endoscopy. Quality indicators to guide endoscopists in the growing field of advanced endoscopy are also underway. We propose that equal attention is given to developing the entire advanced endoscopy team and not the individual endoscopist alone.We suggest that the practice of teams intending to deliver high quality advanced endoscopy is underpinned by six crucial principles concerning: selection, acceptance, complications, reconnaissance, envelopment, and documentation (SACRED).

Background: Patients with T1 colorectal cancer (CRC) often show poor adherence to guideline-recommended treatment strategies after endoscopic resection. To address this challenge and improve clinical decision-making, this study aims to compare the accuracy of surgical management recommendations between large language models (LLMs) and clinicians.
Methods: This retrospective study enrolled 202 patients with T1 CRC who underwent endoscopic resection at three hospitals.
Enterocolitis is a common gastrointestinal manifestation of immune-related adverse events (irAEs); however, only a few studies have reported on irAE enteritis with localized active inflammation in the small intestine. Here, we report the case of a 74-year-old man who developed diarrhea, abdominal pain, and oral intake difficulty and was subsequently hospitalized after receiving atezolizumab for pulmonary adenocarcinoma. Computed tomography and enterocolonoscopy revealed active inflammation in the small intestine but not in the colon, leading to the final diagnosis of irAE enteritis.

Heyde's syndrome signifies the link between aortic stenosis (AS) and bleeding due to angiodysplastic changes. While colonic angiodysplasia is commonly implicated, gastric antral vascular ectasia (GAVE) represents a rarer manifestation in this context. We report the case of a 72-year-old male with severe AS who had recurrent upper gastrointestinal bleeding due to GAVE.

Esophageal-respiratory fistulae are abnormal communications between the esophagus and the respiratory tract, most commonly appearing as tracheoesophageal or bronchoesophageal fistulas. Esophago-pulmonary fistulas represent a rare subtype, typically associated with malignancy, and may lead to severe complications such as lung abscesses. We report a case of a 58-year-old male patient who presented with a two-week history of fever, foul-smelling mucoid sputum, dyspnea, dysphagia, and weight loss.
