Article Abstract

Background: Variants of COL4A1/COL4A2 genes have been reported in fetal intracranial hemorrhage (ICH) cases but their prevalence and characteristics have not been established in a large series of fetuses. Fetal neonatal alloimmune thrombocytopenia is a major acquired ICH factor but the prevalence and characteristics of inherited platelet disorder (IPD) gene variants leading to thrombocytopenia are unknown. Herein, we screened COL4A1/COL4A2 and IPD genes in a large series of ICH fetuses.

Methods: A cohort of 194 consecutive ICH fetuses were first screened for COL4A1/COL4A2 variants. We manually curated a list of 64 genes involved in IPD and investigated them in COL4A1/COL4A2 negative fetuses, using exome sequencing data from 101 of these fetuses.

Result: Pathogenic variants of COL4A1/COL4A2 genes were identified in 36 fetuses (19%). They occurred de novo in 70% of the 32 fetuses for whom parental DNA was available. Pathogenic variants in two megakaryopoiesis genes (MPL and MECOM genes) were identified in two families with recurrent and severe fetal ICH, with variable extraneurological pathological features.

Conclusion: Our study emphasizes the genetic heterogeneity of fetal ICH and the need to screen both COL4A1/COL4A2 and IPD genes in the etiological investigation of fetal ICH to allow proper genetic counseling.

Several new approach methodologies (NAMs) for developmental toxicity (Dev Tox) testing are being used by pharmaceutical companies for derisking or for exploring Dev Tox mechanisms. Regulatory adoption of these NAMs-based approaches as being adequate for Dev Tox risk assessment has been more challenging, due, in part, to dynamic changes in the conceptus and placenta throughout development and the impact of the pharmaceutical on the mother's physiology, which may also have an embryo-fetal impact. Still, there is currently a recognition by Health Authorities that there are certain contexts-of-use under which Dev Tox NAMs can provide information that is adequate to inform risk.

The maternal circulating proteome reflects critical physiological adaptations during pregnancy, yet standardized reference profiles for early gestation are lacking. In this prospective study, we employed targeted liquid chromatography-multiple reaction monitoring-mass spectrometry (LC-MRM-MS) with stable isotope-labeled (SIS) standards to characterize the serum proteome of 83 women with uncomplicated singleton pregnancies between 11 and 13 weeks' gestation. Robust analysis quantified 115 proteins (83% of targets), with 101 meeting ICH M10 standards.

Introduction: Maternal alloimmunization against human platelet antigen-1a (HPA-1a) may lead to severe intracranial hemorrhage (ICH) in the fetus or newborn as a life-threatening complication of fetal neonatal alloimmune thrombocytopenia (FNAIT). Most women who are HPA-1a-alloimmunized do not have a fetus/neonate with ICH. In the absence of predictive tools to identify pregnancies with high risk of ICH outcome, most countries offer weekly antenatal IVIg to all recognized HPA-1a-alloimmunized pregnancies.

Fetal and neonatal alloimmune thrombocytopenia (FNAIT) is a rare but life-threatening condition in which maternal alloantibodies, generated during pregnancy, target human platelet antigens (HPAs) leading to thrombocytopenia and increased risk of bleedings in the fetus or neonate. The most clinically relevant antigen in people of European descent is HPA-1a, located on the integrin β3 subunit. The β3 integrins are conformationally regulated heterodimeric receptors including platelet integrin αIIbβ3, but also αvβ3, expressed strongly on endothelial cells.

Exposure of embryos or fetuses to harmful substances, such as teratogens, can result in embryonic or fetal death and a wide range of malformations. Zebrafish models have emerged as a valuable tool for assessing developmental toxicity and safety profiles of chemical compounds. Our previous research demonstrated that zebrafish larvae exhibit developmental abnormalities that mirror those observed in mammalian studies for more than 80% of the known Reference Compounds listed in the ICH S5 (R3) guideline.
