A 71-year-old Pakistani man with poorly controlled type 2 diabetes mellitus presenting with worsening mental status, abdominal pain, and oral intake for the past seven days was found to have pyogenic hepatic abscess with unculturable bacteria and subsequently found to have rare bacteremia.

We report two cases of Elizabethkingia-related infective endocarditis, highlighting key risk factors such as prosthetic valve replacement. These cases underscore the need to consider endocarditis in the setting of persistent or recurrent bacteremia. Diagnosis proved challenging and required multiple imaging modalities, with positron emission tomography (PET) scan providing critical value when echocardiography was non-contributory.

Background: Surgical management is recommended for infective endocarditis (IE) when there is right heart failure due to severe tricuspid regurgitation, recurrent septic pulmonary emboli, persistent bacteremia, and large tricuspid valve vegetations (≥20 mm). However, sternotomy comes with strict eligibility limitations, including poor functional status, respiratory failure, and recent intravenous drug use.
Case Summary: A 55-year-old woman with a history of intravenous drug use was diagnosed with persistent bacteremia in the setting of tricuspid valve endocarditis.

This study investigated an increase in bacteraemia cases amongst hospitalized patients in Italy during the summer of 2023. To precisely characterize the outbreak, we performed extensive genomic investigation, including both short- and long-read sequencing technologies, combined with bioinformatics analysis. This genomic approach enabled us to identify the putative source of the outbreak and understand the transmission dynamics of this opportunistic pathogen within the hospital.

Bacteremia is a serious clinical condition in which pathogenic bacteria enter the bloodstream, putting patients at risk of septic shock and necessitating aggressive antibiotic treatment. Choosing the most effective antibiotic is crucial not only for resolving the infection but also for minimizing side effects, such as dysbiosis in the healthy microbiome and mitigating the evolution of antibiotic resistance. This requires rapid identification of the pathogen and antibiotic susceptibility testing, yet these processes are inherently slow in standard clinical microbiology labs due to reliance on growth-based assays.
