We developed a metatranscriptomics method that can simultaneously capture the respiratory virome, microbiome, and host response directly from low biomass samples. Using nasal swab samples, we capture RNA virome with sufficient sequencing depth required to assemble complete genomes. We find a surprisingly high frequency of respiratory syncytial virus (RSV) and coronavirus (CoV) in healthy children, and a high frequency of RSV-A and RSV-B co-detections in children with symptomatic RSV. In addition, we have identified commensal and pathogenic bacteria and fungi at the species level. Functional analysis revealed that was highly active in symptomatic RSV subjects. The host nasal transcriptome reveled upregulation of the innate immune system, anti-viral response and inflammasome pathway, and downregulation of fatty acid pathways in children with symptomatic RSV. Overall, we demonstrate that our method is broadly applicable to infer the transcriptome landscape of an infected system, surveil respiratory infections, and to sequence RNA viruses directly from clinical samples.

Rapid antigen diagnostic tests (RDTs) can rapidly detect respiratory pathogens, allowing for the prompt initiation of infection control measures and the prevention of nosocomial spread within hospital settings. In this study, we prospectively evaluated the diagnostic performance of a combined RDT from AllTest Biotech for the simultaneous detection of SARS-CoV-2, influenza virus (IV-A/B), and respiratory syncytial virus (RSV). We compared its diagnostic performance to the Xpert-Xpress-SARS-CoV-2/Flu/RSV molecular test using 100 naso-oropharyngeal swabs (Ct-values ≤ 35), collected from symptomatic patients with acute respiratory tract infections (RTIs) at our tertiary care hospital.

Introduction: Facet joint violation (FJV) is a common complication of intervertebral fusion surgery, altering the load-bearing capability of the facet joints and ultimately contributing to segmental instability. Furthermore, adjacent segment degeneration is one of the potential long-term complications following lumbar spinal intervertebral fusion. For patients with a history of lumbar intervertebral fusion who developed symptomatic spinal stenosis at adjacent segments, adjacent segment decompression surgery is a clinically viable option.

Background: The Pandemic Response Repository through Microbial and Immune Surveillance and Epidemiology (PREMISE) programme was established to translate knowledge gained from global immunoepidemiological surveillance into a better understanding of population-level dynamics of emerging and re-emerging infections, as well as into the discovery and development of biomedical countermeasures against potential pandemic threats. As proof of principle for this approach, we conducted a longitudinal immunoepidemiological study in children in the USA, focusing on enterovirus D68 (EV-D68) infection dynamics but also capturing surveillance of a broad array of other endemic respiratory pathogens. Serendipitously, our sampling spanned the lifting of widespread COVID-19 non-pharmaceutical interventions (NPIs) in 2022-23, following a unique period during which virus exposure markedly diminished.

Objectives: We aimed to describe the incidence, presentation and clinical outcomes of RSV-associated acute respiratory infection (ARI) in older adults using a new national Hospital-based ARI Sentinel Surveillance (HARISS) system in England, prior to RSV vaccine introduction.
Methods: Adults aged ≥65 years from seven hospitals admitted for ≥24 hours with symptomatic ARI were included. We estimated the hospitalisation rate of RSV-associated ARI compared to influenza-associated ARI and assessed clinical outcomes using Poisson regression and mortality using Cox regression.
