Background: CMV serology in the first trimester (T1) of pregnancy relies on IgG and IgM testing, followed by IgG avidity measurement when both IgG and IgM are positive. Valacyclovir is proposed to prevent vertical transmission in maternal primary infection (MPI) during T1.Our objective was to assess the risk of transmission with and without valacyclovir, based on IgG avidity values and MPI timing.

Objective: To evaluate the cost-effectiveness of first trimester single-step universal cytomegalovirus (CMV) serological screening with valacyclovir as vertical transmission prophylaxis versus routine ultrasound-directed testing.
Methods: A payer perspective cost-utility analysis was conducted on a hypothetical population of 100,000 pregnant women for a time horizon of 9 month of full-term pregnancy and the lifetime of children. Using a decision-tree and Markov model, we assessed quality-adjusted life years (QALYs) gained by preventing congenital CMV (cCMV) sequelae against the costs of universal screening and valacyclovir prophylaxis.

Background: The primary goal of periodontology is to prevent tooth loss and reduce the risk of focal infections. Periodontitis lesions can harbor hundreds of thousands of active cytomegaloviruses (virions), which can easily enter the systemic circulation and potentially infect the fetus of a mother with compromised immunity. The healthy, non-inflamed periodontium contains no PCR-detectable cytomegalovirus.
