Article Abstract

Although echocardiography remains the key tool for evaluation of aortic valve stenosis severity, in a fair minority of patients invasive evaluation is still needed. Dual-lumen catheters allow for simultaneous trans-aortic pressure measurements with single arterial access. We describe a technique where traditional hardware using non-dedicated catheters can be used to obtain simultaneous pressure measurements using a 6 French single arterial access.

Perinatal Arterial Ischemic Stroke in Monochorionic Twins: A Retrospective Observational Single-Center Cohort Study.

Stroke

September 2025

Division of Neonatology, Department of Pediatrics, Willem-Alexander Children's Hospital, Leiden University Medical Center, the Netherlands. (B.O.v.O., M.R., M.S.S., E.L., L.S.d.V., S.J.S.).

Background: Monochorionic twins, characterized by placental sharing and vascular anastomoses, carry a high risk of brain injury, including perinatal arterial ischemic stroke (PAIS). However, the pathophysiology and timing-related risk factors of PAIS remain unclear.

Methods: Retrospective cohort of all monochorionic twins with neuroimaging-confirmed PAIS born from 2005 to 2024 and evaluated at a Dutch national referral center.

Background: In critically ill patients with septic shock, adequate oxygenation is crucial and hypoxia should be avoided. However, hyperoxia has been linked to the formation of reactive oxygen species, inflammation, and vasoconstriction, which could potentially harm critically ill intensive care patients. Therefore, this study aimed to examine the association between oxygen exposure and mortality and to define optimal oxygen target ranges for this specific group of patients.

Background: This study aimed to investigate the performance of two versions of ChatGPT (o1 and 4o) in making decisions about coronary revascularization and to compare the recommendations of these versions with those of a multidisciplinary Heart Team. Moreover, the study aimed to assess whether the decisions generated by ChatGPT, based on the internal knowledge base of the system and clinical guidelines, align with expert recommendations in real-world coronary artery disease management. Given the increasing prevalence and processing capabilities of large language models, such as ChatGPT, this comparison offers insights into the potential applicability of these systems in complex clinical decision-making.

Background: Remimazolam tosilate, a novel ultra-short-acting benzodiazepine, demonstrates promising safety profiles in clinical settings. While both remimazolam tosilate and etomidate provide hemodynamic stability during anesthesia induction, limited research has directly compared their effects on electroencephalogram (EEG) burst suppression (periods of transient brain wave silence), a potential predictor of adverse neurological outcomes. This study aims to compare the incidence rate of EEG burst suppression (ESR) with remimazolam tosilate versus etomidate by reviewing the drug regimens used by different anesthesiologists in clinical practice.

Background: Paediatric patients who underwent surgery for mitral regurgitation (MR) have a high risk of recurrence or death; however, no prediction tool has been developed to risk-stratify this challenging subpopulation.

Methods: In this multicentre cohort study, paediatric patients undergoing surgery for congenital MR in Shanghai Children's Medical Center in January 1st, 2009-December 31st, 2022 were included for analysis while those had a combination with infective endocarditis, anomalous left coronary artery from the pulmonary artery, rheumatic valvular disease, connective tissue disease, or single ventricle were excluded. A Cox regression model predictive of the primary outcome (a composite of mortality or mitral valve [MV] re-operation) was derived and converted to a point-based risk score.
