Patients with Fontan circulation are increasingly presenting for non-cardiac surgical procedures in adulthood, often involving complex anaesthetic management due to their unique physiology. We report the case of a 39-year-old Fontan patient who underwent elective thoracoscopic epicardial pacemaker lead revision under one-lung ventilation, requiring advanced hemodynamic monitoring, inotropic support, and pulmonary vasodilators. This case highlights the significant perioperative challenges in such patients and the need for a specialised, multidisciplinary team.

This case report presents the successful surgical management of a 34-year-old male who had blunt chest trauma resulting in a left atrial appendage tear after a traffic accident. He arrived at the emergency department 30 minutes post-injury in a gasping state, showing signs of hypovolemic shock, tachycardia (132 bpm), hypotension (blood pressure 88/48 mm Hg) and decreased breath sounds on the left side. Objectives: Assessing surgical trainees' performance requires objective, validated, and reliable evaluation tools. We developed and validated the Lung Lobectomy Performance Assessment Scale Score (LOB-PASS) to objectively evaluate lung lobectomy performance, including lymph node dissection, across different surgical approaches (postero-lateral thoracotomy, video-assisted, and robot-assisted).
Methods: The LOB-PASS scale was developed using the Delphi consensus method and response process testing in simulation sessions.
