Background: Previous studies showed that recovered coronavirus disease 2019 (COVID-19) patients can have a subsequent positive polymerase chain reaction (PCR) test for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) after they are discharged from the hospital. Understanding the epidemiological characteristics of recovered COVID-19 patients who have a re-positive test is vital for preventing a second wave of COVID-19.
Methods: This retrospective study analyzed the epidemiological and clinical features of 20,280 COVID-19 patients from multiple centers in Wuhan who had a positive PCR test between December 31, 2019, and August 4, 2020. The RT-PCR test results for 4079 individuals who had close contact with the re-positive cases were also obtained.
Results: In total, 2466 (12.16%) of the 20,280 patients had a re-positive SARS-CoV-2 PCR test after they were discharged from the hospital, and 4079 individuals had close contact with members of this patient group. All of these 4079 individuals had a negative SARS-CoV-2 PCR test.
Conclusions: This retrospective study in Wuhan analyzed the basic characteristics of recovered COVID-19 patients with re-positive PCR test and found that these cases may not be infectious.

Purpose Of Review: Diagnostic stewardship (DS) aims to optimise the use of laboratory testing to improve patient care while reducing unnecessary tests. This review examines recent evidence on DS interventions to optimise the use of resources, focusing on three key areas: reducing unnecessary testing, maximising the impact of existing tests, and avoiding the overdiagnosis of hospital-acquired infections.
Recent Findings: Multiple interventions have demonstrated effectiveness in reducing unnecessary blood and urine culture testing, including clinical decision support tools, education programs, and multidisciplinary approaches.

Rheumatoid arthritis (RA) is a chronic autoimmune disease in which dysregulated interferon regulatory factor 5 (IRF5) may amplify pro-inflammatory pathways; prior genetic studies of IRF5 single-nucleotide variants (SNVs) in RA are inconsistent across populations and have not included mestizo Mexicans or evaluated rs59110799 in RA. We aimed to test whether four IRF5 SNVs (rs2004640G/T, rs2070197T/C, rs10954213G/A, rs59110799G/T) confer susceptibility to RA in women from Central Mexico. In a case-control study of 239 women with RA and 231 female controls (all self-identified Mexican-Mestizos, ≥3 generations), genotyping was performed by real-time PCR with TaqMan® probes; 80% of samples were duplicated (100% concordance) and control genotypes conformed to Hardy-Weinberg equilibrium.

Donor-derived cell-free DNA (dd-cfDNA) has emerged as a valuable noninvasive biomarker for detecting allograft injury in solid organ transplantation. It is released into the bloodstream from the transplanted organ as a result of cell injury and immune activation, with baseline levels influenced by organ type, tissue turnover, and posttransplant physiological changes. Several analytical platforms are available, including quantitative polymerase chain reaction (PCR), digital droplet PCR, and next-generation sequencing, each differing in sensitivity, throughput, and reporting format.
