Deficiency of ASM (acid sphingomyelinase) causes the lysosomal storage Niemann-Pick disease (NPD). Patients with NPD type B may develop progressive interstitial lung disease with frequent respiratory infections. Although several investigations using the ASM-deficient (ASMKO) mouse NPD model revealed inflammation and foamy macrophages, there is little insight into the pathogenesis of NPD-associated lung disease. Using ASMKO mice, we report that ASM deficiency is associated with a complex inflammatory phenotype characterized by marked accumulation of monocyte-derived CD11b macrophages and expansion of airspace/alveolar CD11c CD11b macrophages, both with increased size, granularity, and foaminess. Both the alternative and classical pathways were activated, with decreased phagocytosis of opsonized (Fc-coated) targets, preserved clearance of apoptotic cells (efferocytosis), secretion of Th2 cytokines, increased CD11c/CD11b cells, and more than a twofold increase in lung and plasma proinflammatory cytokines. Macrophages, neutrophils, eosinophils, and noninflammatory lung cells of ASMKO lungs also exhibited marked accumulation of chitinase-like protein Ym1/2, which formed large eosinophilic polygonal Charcot-Leyden-like crystals. In addition to providing insight into novel features of lung inflammation that may be associated with NPD, our report provides a novel connection between ASM and the development of crystal-associated lung inflammation with alterations in macrophage biology.

Background: Online postal self-sampling (OPSS) allows service users to screen for sexually transmitted infections (STIs) by ordering a self-sampling kit online, taking their own samples, returning them to a laboratory for testing, and receiving their results remotely. OPSS availability and use has increased in both the United Kingdom and globally the past decade but has been adopted in different regions of England at different times, with different models of delivery. It is not known why certain models were decided on or how implementation strategies have influenced outcomes, including the sustainability of OPSS in sexual health service delivery.

Acute lung injury (ALI) is characterized by the excessive accumulation of reactive oxygen species (ROS), which triggers a severe inflammatory cascade and the destruction of the alveolar-capillary barrier, leading to respiratory failure and life-threatening outcomes. Considering the limitations and adverse effects associated with current therapeutic interventions, developing effective and safe strategies that target the complex pathophysiological mechanisms of ALI is crucial for improving patient outcomes. Herein, we developed an inhalable, multifunctional nanotherapeutic (MSCNVs@CAT) by encapsulating catalase (CAT) in mesenchymal-stem-cell-derived nanovesicles (MSCNVs).

Background: Research on Post-acute sequelae of COVID (PASC) has focused on the prevalence of symptoms, leaving gaps in our understanding of predictors of health care seeking.
Objective: To identify clinical and sociodemographic characteristics associated with PASC care seeking.
Methods: Retrospective cohort study of adult patients with COVID-19 diagnosis between January 1, 2021 and June 30, 2022 in a community-based comprehensive health care delivery system at 21 hospitals and medical clinics in Northern California.

A comprehensive understanding of the factors influencing the epidemiological dynamics of COVID-19 across the pandemic waves-particularly in terms of disease severity and mortality-is critical for optimizing healthcare services and prioritizing high-risk populations. Here we aim to analyze the factors associated with short-term and prolonged hospitalization for COVID-19 during the first three pandemic waves. We conducted a retrospective observational study using data from individuals reported in the e-SUS-VS system who were hospitalized for COVID-19 in a state in a southeast state of Brazil.

causes otitis media and severe diseases including pneumonia, meningitis and bacteraemia. The rise of antimicrobial resistance (AMR) in , facilitated by mobile genetic elements (MGEs), complicates infection treatment. While pneumococcal conjugate vaccine (PCV) deployment has reduced disease burden, non-vaccine serotypes (NVTs) have increased and now cause invasive disease.
