Acute stroke patients transferred to thrombectomy capable centers (TCC), undergo a CT head exam upon arrival at the TCC to evaluate for ASPECTS decay and intracranial hemorrhage. In patients who received iodinated contrast prior to transfer, parenchymal enhancement may simulate hemorrhage on this post-transfer CT. We report two cases utilizing CT spectral imaging to differentiate between parenchymal contrast enhancement and hemorrhage in this setting. TCC may consider dual-energy or dual-layer (spectral) imaging for this patient cohort.

Radical esophagectomy remains the cornerstone of curative treatment for esophageal cancer, but is frequently complicated by postoperative events, most notably anastomotic leakage. Anastomotic leakage, occurring in up to 30% of cases, is multifactorial in origin and significantly increases morbidity and mortality. This review aims to summarize current management strategies, highlight emerging therapies, and identify persistent clinical challenges related to this complication.

The nanoscale spatial arrangement of T cell receptor (TCR) ligands critically influences their activation potential in CD8 T cells, yet a comprehensive understanding of the molecular landscape induced by engagement with native peptide-MHC class I (pMHC-I) remains incomplete. Using DNA origami nanomaterials, we precisely organize pMHC-I molecules into defined spatial configurations to systematically investigate the roles of valencies, inter-ligand spacings, geometric patterns, and molecular flexibility in regulating T cell function. Developing artificial hosts with temperature-driven conformational switching behaviors facilitates our understanding of the temperature-dependent allostery and adaptation mechanisms in natural recognition systems. Herein, we report the design and synthesis of three pairs of water-soluble, enantiomeric binaphthalene-based tetraimidazolium macrocycles (SS/RR-1•4Cl- - SS/RR-3•4Cl-) as artificial hosts for exploring sequence-selective recognition of dinucleotides in aqueous media. Owing to the reversible rotational conformation of axially chiral binaphthyl units, SS-1•4Cl- demonstrates the conformational switching, converting from cis-conformation (SS-1) to trans-conformation (SS-1) by increasing temperature, thereby causing the recognition cavity to transition from a closed to an open state.
