A thyroid storm is a medical emergency that requires high clinical suspicion and emergency treatment. We report an unusual case of thyroid storm and agranulocytosis managed with lithium therapy. The patient is a 32-year-old woman with a history of Graves' disease on methimazole therapy who presented to the emergency department with fever and palpitations.

Human aging presents an evolutionary paradox: while aging rates remain constant, healthspan and lifespan vary widely. We address this conundrum via salutogenesis-the active production of health-through immune resilience (IR), the capacity to resist disease despite aging and inflammation. Analyzing ~17,500 individuals across lifespan stages and inflammatory challenges, we identified a core salutogenic mechanism: IR centered on TCF7, a conserved transcription factor maintaining T-cell stemness and regenerative potential.

This perspective lays out the challenges and conundrums facing global health and discusses possible solutions applicable in the future. The world is facing numerous challenges that include those associated with globalization, climate change, emerging diseases, continuation of non-communicable diseases, reemerging communicable diseases, antimicrobial resistance (AMR), wars, terrorism, and humanitarian crises, among others. The recent challenges exaggerated by the COVID-19 pandemic have exposed vulnerabilities within healthcare systems, particularly in low- and middle-income countries (LMIC).

Maintaining stable hydric and thermal states are dual challenges for reptiles that inhabit terrestrial environments with variable conditions across time and space. Under some conditions, reptiles face a conundrum where both physiological parameters cannot be simultaneously maintained at preferred levels by behavioral or physiological means. Prioritization of behavioral regulation of hydric or thermal state, and at which point this prioritization changes, was tested for nine species of congeneric tropical geckos by assessing their use of microhabitats with distinct thermal and hydric conditions in a controlled environment.
