Globally, thyroid cancer accounts for 2 % of all cancer diagnoses, and can be classified as well-differentiated or undifferentiated. Currently, differentiated thyroid carcinomas have good prognoses, and can be treated with a combination of therapies, including surgical thyroidectomy, radioactive iodine therapy and hormone-based therapy. On the other hand, anaplastic thyroid carcinoma, a subtype of undifferentiated thyroid carcinoma characterized by the loss of thyroid-like phenotype and function, does not respond to either radioactive iodine or hormone therapies. In most cases, anaplastic thyroid carcinomas are diagnosed in later stages of the disease, deeming them inoperable, and showing poor response rates to systemic chemotherapy. Recently, treatment courses using multiple-target agents are being explored and clinical trials have shown very promising results, such as overall survival rates, progression-free survival and tumor shrinkage. This review is focused on thyroid carcinomas, with particular focus on anaplastic thyroid carcinoma, exploring its undifferentiated nature. Special interest will be given to the treatment approaches currently available and respective obstacles or drawbacks. Our purpose is to contribute to understand why this malignancy presents low responsiveness to current treatments, while overviewing novel therapies and clinical trials.

The Casearia genus (Salicaceae) is well known as a source of natural antitumor prototypes, as described for clerodane diterpenes. In this way, the dichloromethane phase from leaves of Casearia arborea was phytochemically evaluated in liquid chromatography coupled to high-resolution mass spectrometry, furnishing flavonoid, flavolignan, and phenylpropanoid derivatives, and apocarotenoid terpenoids according to Global Natural Products Social Molecular Networking (GNPS) dereplication. The flavolignans were chemically monitored by liquid chromatography using a UV-Vis detector and purified by a semi-preparative liquid chromatographic system, furnishing tricin, tricin-4″-O-(threo-guaiacylglyceryl) ether (salcolin A), and tricin-4″-O-(erythro-guaiacylglyceryl) ether (salcolin B).

Anaplastic thyroid carcinoma (ATC) is one of the most lethal human malignancies, often evolving from differentiated thyroid carcinoma (DTC) through a poorly understood dedifferentiation process. To elucidate this transition, spatial transcriptomic sequencing (spRNAseq) is performed on seven samples containing coexisting regions of ATC, poorly differentiated thyroid carcinoma, and DTC. SpRNAseq revealed that ATC regions were characterized by upregulated genes involved in immune suppression, angiogenesis, and extracellular matrix remodeling.

Although a diagnosis of anaplastic thyroid carcinoma (ATC) can be rendered on fine needle aspiration (FNA), a core needle biopsy is often performed to provide sufficient material for immunohistochemical and molecular analysis. Rendering an ATC diagnosis on core biopsy can be challenging due to limited material. It is crucial that other diagnostic entities in the differential, such as poorly differentiated thyroid carcinoma, medullary thyroid carcinoma, lymphoma, metastases, and NUT carcinoma (among others), are considered and that immunohistochemistry (IHC) is employed judiciously to support the diagnosis.
