Article Abstract

A wide clinical spectrum has become a hallmark of the SARS-CoV-2 (COVID-19) pandemic, although its immunologic underpinnings remain to be defined. We have performed deep characterization of B cell responses through high-dimensional flow cytometry to reveal substantial heterogeneity in both effector and immature populations. More notably, critically ill patients displayed hallmarks of extrafollicular B cell activation as previously described in autoimmune settings. Extrafollicular activation correlated strongly with large antibody secreting cell expansion and early production of high levels of SARS-CoV-2-specific antibodies. Yet, these patients fared poorly with elevated inflammatory biomarkers, multi-organ failure, and death. Combined, the findings strongly indicate a major pathogenic role for immune activation in subsets of COVID-19 patients. Our study suggests that, as in autoimmunity, targeted immunomodulatory therapy may be beneficial in specific patient subpopulations that can be identified by careful immune profiling.

Vaccines effectively stimulate protective immune responses in healthy individuals, but the precise roles of germinal center (GC) and follicular helper T (TFH) cells in SARS-CoV-2 vaccine responses are not fully understood. This study used a conditional loss-of-function mouse model to investigate antibody responses to the Wuhan spike protein, specifically eliminating newly developed TFH cells during either the primary or memory phase. Our findings demonstrated that TFH-mediated GC responses are essential for primary vaccination.

Objectives: This study aims to identify salivary gland (SG) transcriptomic signatures associated with peripheral and histological biomarkers of disease activity and lymphoma risk factors to inform on Sjögren's disease (SjD) stratification.

Methods: Bulk RNA-sequencing of labial SG from exploratory Queen Mary University of London (QMUL) cohort (SjD [n = 55] and non specific-chronic-sialadenitis [sicca, n = 44]) and trial for anti-B-cell therapy in Sjogren's syndrome (TRACTISS)validation cohort (SjD, n = 29) analysed integrating transcriptomic, clinical, serological, and histological data.

Results: Unsupervised gene clustering confirmed clear transcriptome segregation between sicca and SjD.

The involvement of T cells, particularly cytotoxic T cells, in tumor immunity has been reported, but the roles of B cells and antibodies in tumor immunity remain unclear. Previous studies have primarily focused on circulating B cells in the blood, with limited investigation of tumor-infiltrating B cells (TIL-Bs) and their secreted antibodies in head and neck squamous cell carcinoma (HNSCC) tissue. This study aimed to clarify TIL-Bs' role in HNSCC through comprehensive analyses including single-cell RNA sequencing of 68 cases, B-cell receptor repertoire analysis, multiplex immunofluorescence staining, and spatial transcriptomics analysis of resected HNSCC specimens.

Unlabelled: Many patients with common variable immunodeficiency (CVID), including those with CTLA4 deficiency, variants and activated PI3K-delta syndrome (APDS), develop autoimmunity that is refractory to treatment. Despite this shared clinical phenotype, a unifying mechanism for the breakdown of B cell tolerance across monogenic forms of CVID has not been established. Here, we demonstrate that patients with loss-of-function variants, like those with variants and APDS, exhibit dysregulated CD4 T cell expansion, accumulation of transitional B cells, and a relative lack of follicular B cells.
