Introduction: Fluoroless mapping and ablation using a Pentaspline pulsed field ablation catheter has many advantages. This can be achieved using a "tripolar configuration," which enables high-quality electroanatomical maps, improves the ability to localize electrograms (EGMs), and minimizes the use of additional mapping catheters compared to the standard bipolar configuration. We aimed to evaluate the benefits of using a tripolar configuration in fluoroless atrial fibrillation ablation compared to the standard bipolar configuration.

Background And Aims: Ablation for premature ventricular complexes (PVCs) originating from the right ventricular inflow tract (RVIT) is challenging. Few studies have identified the correlation between right ventricular false tendons (RVFTs) and RVIT PVCs. This study aimed to verify RVFTs as arrhythmogenic and electro-anatomical substrates for PVCs, and propose an enlightening mapping and ablation protocol to improve operative efficacy.

Background: Kimura disease is a rare inflammatory condition that typically manifests with subcutaneous nodules of the head and neck. This is the first documented case of intracardiac Kimura disease.
Case Summary: A 57-year-old woman presented with a heart murmur and dyspnea.

Background: Intracardiac echocardiography (ICE)-guided non/minimized-fluoroscopy catheter ablation for atrial fibrillation (AF) has been reported, but its effectiveness and safety still lack multicenter evidence.
Objectives: The authors sought to evaluate the effectiveness and safety of ICE-guided non/minimized-fluoroscopy catheter ablation compared with the traditional fluoroscopy-guided approach in patients with paroxysmal AF.
Methods: A total of 448 patients with paroxysmal AF, from 15 centers in China, were randomly assigned in a 1:1 ratio to a non/minimized-fluoroscopy group (n = 223) and a traditional approach group (n = 225).

Intracardiac echocardiography can be performed under local anesthesia and provides real-time monitoring with high image quality without interference from the procedure during transcatheter aortic valve replacement, even when transthoracic echocardiography and transesophageal echocardiography guidance are not appropriate owing to the patient's severe chronic obstructive pulmonary disease.
