This case describes the surgical repair of critical limb ischemia in a patient with diffuse multilevel peripheral arterial disease. It demonstrates the value of patient-specific approaches that employ hybrid endovascular and open surgical techniques to reconstruct blood flow in patients who are not ideal candidates for traditional revascularization. We detail a technique that combines endarterectomy, femoropopliteal bypass, angioplasty, and stenting. This case suggests that innovative hybrid approaches can be used to achieve limb salvage in some patients with multilevel peripheral vascular disease who would otherwise undergo primary amputation.

Objectives: The purpose of this paper was to compare the efficacy of covered stents (CSs) and bare metal stents (BMSs) in treating all types of aortoiliac occlusive disease (AIOD) and subsequently to analyze the risk factors associated with restenosis, limb salvage, and patency.
Methods: This prospective cohort study included consecutive patients with AIOD who underwent aortoiliac angioplasty, and two groups of patients were evaluated: patients with AIOD submitted to endovascular treatment with the use of covered stents and bare metal stents. Patients with critical limb ischemia or incapacitating claudication who underwent aortoiliac angioplasty during the index period were eligible for the study.

This case report discusses the overall care of a female patient with nephrotic syndrome secondary to non-pre-eclampsia-related hypertension in pregnancy, emphasizing the challenges and multidisciplinary treatment needed for desired results. The case presented here involves a 32-year-old pregnant woman with a history of unexplained primary infertility who conceived through in vitro fertilization (IVF). At 26 weeks and three days of gestation, she presented with symptoms suggestive of nephrotic syndrome, including lower limb swelling, facial puffiness, oliguria, and dark-colored urine.

CT angiography (CTA) of the aortoiliofemoral (AIF) arteries in the abdomen, pelvis, and lower extremities has become an invaluable tool in assessment of patients with peripheral arterial disease (PAD) and lower extremity trauma. AIF CTA provides rapid and comprehensive assessment of arterial inflow and outflow, guiding management of patients with chronic claudication and those with more acute manifestations, including atherothrombotic occlusion, embolic disease, or thrombosis of prior interventions such as bypass graft or stent placement. Careful attention to technique is critical in performing diagnostic AIF CTA, as pitfalls related to imaging too early or too late relative to the arrival of contrast material in the legs can lead to misdiagnosis or diagnostic uncertainty.
