Afferent loop obstruction (ALO) caused by cancer recurrence after total gastrectomy (TG) can be managed by either surgical or non-surgical treatment. The general condition of patients with recurrent gastric cancer is often poor, so a less invasive non-surgical treatment is desirable. We report the case of a 75-year-old male who had undergone TG for gastric cancer 6 months previously and who presented at our hospital with abdominal pain and vomiting. Abdominal computed tomography scan showed a dilated afferent loop, and additionally a low-density lesion around jejunojejunal anastomosis, suggesting that ALO is associated with peritoneal recurrence. A self-expandable metal stent (SEMS) was endoscopically placed to treat ALO after decompression of the dilated afferent loop using an intestinal tube. He retained a good quality of life until his death due to cancer progression 5 months after the SEMS placement. Our case indicates that SEMS could be a less invasive alternative to surgery, and may confer a better quality of life for patients with ALO due to cancer recurrence after TG. This is the valuable report of case in which endoscopic metallic stent placement succeeded for ALO caused by peritoneal recurrence after TG.

It is not just a sending and receiving channel, but a living structure capable of autonomously processing the afferent and efferent notifications with which it comes into contact. The osteopathic neurological model includes the concept of facilitation of the spinal segment, that is, a reflex arc that is established in a spinal segment between two visceral and/or somatic structures, creating a loop of chronicity.

Endoscopic ultrasonography (EUS) has progressed beyond diagnostic imaging to include EUS-guided tissue acquisition and EUS-directed therapies. This review provides an update on EUS-guided drainage and anastomotic procedures, and other therapeutic procedures. Today, EUS-guided drainage of symptomatic walled-off pancreatic fluid collections is the norm, with endoscopic necrosectomy as an adjunct.

The muscle-tendon complex (MTC) in biological systems integrates contractile actuation and proprioceptive sensing, enabling coordinated feedback control of muscle activations through simultaneous afferent (sensory) and efferent (motor) signaling. To achieve similar functionality, artificial muscles, often based on polymeric materials with intricate material behaviors, require embedded proprioceptive capabilities to enable adaptive and reliable feedback control. Here, an artificial MTC-inspired liquid crystal elastomer (LCE) muscle with embedded physical intelligence is presented that supports simultaneous sensing and actuation.

We report the case of a 58-year-old male with prior Billroth II gastrectomy who presented with worsening abdominal pain. Contrast-enhanced CT revealed a hyperdense, corticated, triangular-shaped foreign body (FB), consistent with a chicken bone, which had undergone retrograde migration and become impacted in the afferent duodenal loop. The FB's orientation and location were precisely characterized, with associated inflammatory changes but no perforation.
