In this issue of Neuron, Harris et al. (2018) show that a signal transduction pathway normally exploited by the innate immune system in recognizing foreign agents plays a critical role in controlling a synapse's ability to maintain stability in the efficacy of synaptic transmission over both rapid and prolonged timescales.

Cotranslational protein folding follows a distinct pathway shaped by the vectorial emergence of the peptide and spatial constraints of the ribosome exit tunnel. Variations in translation rhythm can cause misfolding linked to disease; however, predicting cotranslational folding pathways remains challenging. Here, we computationally predict and experimentally validate a vectorial hierarchy of folding resolved at the atomistic level, where early intermediates are stabilized through non-native hydrophobic interactions before rearranging into the native-like fold.

Understanding their homeostatic regulation is essential for developing therapeutic strategies. Background: Emerging adults living with type 1 diabetes (T1D) need targeted support to equip them with the knowledge and motivation required for self-management, particularly as they transition from pediatric to adult care. While multicomponent digital health interventions have shown promise in addressing their multifaceted needs, traditional effectiveness studies provide little, if any, insights into which components work effectively, how they function, and for whom.
Objective: This study aims to explore the implementation of a multicomponent, text message-based digital intervention (Keeping in Touch; KiT) to provide early insights into which components may shape participants' transition experiences and how.

Background: Transcatheter Aortic Valve Implantation (TAVI) has changed the treatment paradigm of the aortic stenosis (AS). It has become the treatment of choice in patients with symptomatic AS and surgical high risk, and a valid alternative to surgical aortic valve replacement in patients with low and medium surgical risk. Despite numerous evidence on clinical results, indications and benefits, only a few studies analyse it from patient's perspective and the impact of TAVI on them.

The ease with which microRNA inhibitors (antimiRs) can be delivered varies with the intended target cell type and tissue. AntimiRs are of interest as potential therapeutics for kidney conditions, including ischaemia-reperfusion injury in transplantation. During ex-situ human kidney perfusion, antimiRs are delivered to the proximal tubule epithelium without the use of transfection reagents by an endocytic process.
