Diabetes mellitus (DM) is a serious metabolic disorder affecting millions of people worldwide. The high rate of mortality and morbidity during DM is attributed to the increased atherothrombotic events due to platelet activation and apoptosis leading to macro and micro vascular occlusions. The platelet hyper-reactivity and apoptosis during DM is accounted for the accumulated reactive oxygen species (ROS) due to increased aldose reductase (AR) and NADPH oxidase (NOX) activities. Considering aspirin insensitivity in DM patients, new therapies targeting the underlying mechanism is urgently warranted. Berberine, a benzylisoquinoline alkaloids, from Chinese folk medicine has been demonstrated with several anti-diabetic effects. Therefore, we evaluated whether berberine inhibits high glucose potentiated platelet aggregation, apoptosis and further evaluated the mechanism of its action in platelets. Berberine was found to inhibit platelet aggregation, superoxide production via modulating AR, NOX, and glutathione reductase activities in high glucose (HG) treated platelets. Correlated with this, berberine inhibited, calcium release, ERK activation, α- and dense granule release and platelet adhesive properties. In addition, berberine inhibited p38-p53 mediated BAX activation, mitochondrial dysfunction and platelet apoptosis induced by HG. The platelet protective effect of berberine by inhibiting AR and NOX in high glucose-treated platelets suggest that berberine could be developed as a potential therapeutic molecule in the treating pathologies associated with DM.

Background And Objectives: The US Food and Drug Administration guidance for cold-stored platelets (CSPs) permits storage of apheresis platelets at 1-6°C for ≤14 days. During a pilot programme, CSPs were evaluated in a large US blood centre over a 10-month period (September 2023 to July 2024) to better understand the formation of aggregates under routine use.
Materials And Methods: Platelets collected in 100% plasma were moved into cold storage within 4 h of collection and shipped to local hospitals.

Platelet integrin αIIbβ3 is the final common effector of arterial thrombosis: it switches from a low-affinity to a high-affinity state, binds fibrinogen, and initiates the outside-in signals that stabilize a growing clot. Calcium- and integrin-binding protein 1 (CIB1) emerged as the first endogenous partner of the αIIb cytoplasmic tail and is now recognized as a dual-role adaptor. At rest, Ca-free CIB1 tethers the inner membrane clasp and restrains premature integrin activation; after ligand engagement, Ca-bound CIB1 docks onto αIIb, recruits focal-adhesion kinase and amplifies Src-dependent cytoskeletal remodeling.

Glanzmann thrombasthenia (GT) is a rare autosomal recessive platelet disorder characterized by abnormalities in platelet aggregation, resulting from quantitative or qualitative defects in integrins αIIb and β3. Currently, allogeneic hematopoietic stem cell transplantation (allo-HSCT) is the only potentially curative therapeutic approach for severe GT. In this report, we present 2 children with GT that underwent successful allo-HSCT, along with 2008 to 2022 data from the Center for International Blood and Marrow Transplant Research and a summary of the existing literature providing further evidence that allo-HSCT can be a curative approach that prevents severe and life-threatening bleeding in GT.

The vascular endothelium is responsible for regulating vascular tone, maintaining fluid homeo-stasis, and preventing platelet aggregation, exhibits regulatory properties in vasorelaxation and vasoconstriction - it produces, among others, nitric oxide and endothelin. The imbalance of vasoactive molecules leads to the loss of their function, known as endothelial dysfunction. Impaired endothelial function is observed in people with metabolic disorders, often preceding the onset of the disease by several years.
