Background: Timely extubation is essential in ICU patients, yet traditional predictors such as the rapid shallow breathing index (RSBI) have limited accuracy. Diaphragm and lung ultrasound offer promising, non-invasive alternatives for assessing extubation readiness.

Because obstructive sleep apnoea (OSA) is a chronic condition, and requires a continuous treatment and monitoring of therapy success, telematic communications could be useful to establish diagnostic and therapeutic strategies. The differential Fick method is well established for measuring effective pulmonary blood flow (EPBF) and cardiac output (CO) but until now it has only been used for patients on mechanical ventilation. Here we present and evaluate a new approach adapted to spontaneous breathing situations. Ten healthy subjects with diverse anthropometric and respiratory parameters were studied in the sitting position.

Functional thoracic MRI provides regional assessment of the three principal components of lung function: ventilation, perfusion, and gas exchange. It offers advantages over pulmonary function tests like spirometry, which yield only global measurements. MRI enables comprehensive evaluation of respiratory mechanics, including chest wall and diaphragm motion, dynamic large airway instability, and lung ventilation using various contrast mechanisms and gas agents.
