Article Abstract

Unlabelled: IntroductionDespite improvements in the medical and surgical management of infants with CHD, growth failure before surgery in many infants continues to be a significant concern. A nutritional pathway was developed, the aim of which was to provide a structured approach to nutritional care for infants with CHD awaiting surgery.Materials and methodsThe modified Delphi process was development of a nutritional pathway; initial stakeholder meeting to finalise draft guidelines and develop questions; round 1 anonymous online survey; round 2 online survey; regional cardiac conference and pathway revision; and final expert meeting and pathway finalisation.

Results: Paediatric Dietitians from all 11 of the paediatric cardiology surgical centres in the United Kingdom contributed to the guideline development. In all, 33% of participants had 9 or more years of experience working with infants with CHD. By the end of rounds 1 and 2, 76 and 96% of participants, respectively, were in agreement with the statements. Three statements where consensus was not achieved by the end of round 2 were discussed and agreed at the final expert group meeting.

Conclusions: Nutrition guidelines were developed for infants with CHD awaiting surgery, using a modified Delphi process, incorporating the best available evidence and expert opinion with regard to nutritional support in this group.

Background: Congenital heart disease (CHD) is the most common anatomical malformation among live-born infants and has been linked to various prenatal factors. Maternal sleep disturbances, a common issue during pregnancy, have been associated with adverse pregnancy outcomes and neonatal health problems. However, epidemiological evidence on the association between maternal sleep disturbances and the risk of CHD in offspring remains limited and inconclusive.

Capillary leak syndrome is a serious postoperative complication in neonates and infants after open-heart surgery for CHD. The aim of this study was to investigate the relationship between sternal opening width and the occurrence of capillary leak syndrome in patients undergoing cardiac surgery with delayed sternal closure. We retrospectively analysed the clinical data of neonates and infants (aged < 12 months) who underwent open-heart surgery with delayed sternal closure at our institution between January 2016 and December 2021.

Background: Prenatal detection rates of congenital heart disease (CHD) in patients requiring early surgical intervention have been suboptimal, with variability between geographic regions and across defect types. In 2013, obstetric screening ultrasound guidelines were updated, recommending the addition of outflow tract views to improve prenatal detection rates. The objective of this study was to evaluate prenatal detection rates before and after the introduction of outflow tract views in obstetric screening examinations.

Objectives: Describe the hemodynamic implications of anaesthetic choice among children with heart disease undergoing cardiac catheterisation.

Methods: Study 1 was a secondary analysis of data obtained during catheterisation-based hemodynamic assessment of infants with hypoplastic left heart syndrome following Stage 1 palliation, randomised in the Single Ventricle Reconstruction trial. Measured and calculated hemodynamics including pulmonary and systemic vascular resistance indexed to body surface area (PVRi and SVRi respectively) and pulmonary/systemic blood flow (Qp/Qs) were analysed with respect to anaesthetic employed during catheterisation, classified as moderate sedation or general anaesthesia.

Background: Congenital heart disease (CHD) is a leading cause of infant mortality. Its treatment involves a complex "continuum of care" over time and distance, the equitable delivery of which is particularly challenging in low- and middle-income countries that lack infrastructure, a specialized workforce, or a coordinated patient throughput system. We report an update on the impact of a population-based program, Hridyam (established 2017), to address gaps and inequity in CHD care in Kerala, India.
