Article Abstract

There is a renewed interest for β-lactams for treating infections due to Mycobacterium tuberculosis and M. abscessus because their β-lactamases are inhibited by classical (clavulanate) or new generation (avibactam) inhibitors, respectively. Here, access to an azido derivative of the diazabicyclooctane (DBO) scaffold of avibactam for functionalization by the Huisgen-Sharpless cycloaddition reaction is reported. The amoxicillin-DBO combinations were active, indicating that the triazole ring is compatible with drug penetration (minimal inhibitory concentration of 16 μg mL for both species). Mechanistically, β-lactamase inhibition was not sufficient to account for the potentiation of amoxicillin by DBOs. Thus, the latter compounds were investigated as inhibitors of l,d-transpeptidases (Ldts), which are the main peptidoglycan polymerases in mycobacteria. The DBOs acted as slow-binding inhibitors of Ldts by S-carbamoylation indicating that optimization of DBOs for Ldt inhibition is an attractive strategy to obtain drugs selectively active on mycobacteria.

Background/objectives: is a frequent causative agent of urinary and wound infections in both community and hospital settings. It develops resistance to expanded-spectrum cephalosporins (ESCs) due to the production of extended-spectrum β-lactamases (ESBLs) or plasmid-mediated AmpC β-lactamases (p-AmpCs). Recently, carbapenem-resistant isolates of emerged due to the production of carbapenemases, mostly belonging to Ambler classes B and D.

Klebsiella pneumoniae carbapenemase (KPC) is a frequent and widespread carbapenemase, with over 260 variants identified. While KPC often evolves resistance to ceftazidime-avibactam, cefiderocol remains a key treatment option. Some variants, such as KPC-33 (D179Y), reduce cefiderocol susceptibility, but typically with only modest MIC increases.

What Is Already Known About This Topic?: Carbapenem-resistant (CRKP) poses a major threat to global health. The co-production of multiple carbapenemases has emerged as a critical concern, further limiting the effectiveness of last-resort antibiotics such as ceftazidime-avibactam.

What Is Added By This Report?: This study identifies an IncP6 plasmid co-harboring both and in a clinical isolate of .

We characterized all ceftazidime-avibactam-resistant KPC-producing K. pneumoniae (KPC-Kp) isolates recovered from both patients and environmental samples at the ICU of our hospital in 2020, during the COVID-19 pandemic initiation. Antimicrobial susceptibility testing (Sensititre-EUMDROXF; disk-diffusion) and WGS analysis (Illumina-Novaseq/Miseq; Oxford Nanopore-MinION) were performed.

Department of Experimental Medicine, University of Salento, Via Provinciale Monteroni n. 165, Lecce 73100, Italy.

Introduction: Multi-carbapenemases producing Enterobacterales (MCP-EB) represents a new public health challenge due to their ability to display complex resistance phenotypes.

Methods: MCP-EB isolates identified from patients admitted to an Italian Center in the period 2020-2024 were included. Clinical features were collected.
