The American Academy of Otolaryngology-Head and Neck Surgery Foundation has published a supplement to this issue of Otolaryngology-Head and Neck Surgery featuring the "Clinical Practice Guideline: Benign Paroxysmal Positional Vertigo (Update)." To assist in implementing the guideline recommendations, this article summarizes the rationale, purpose, and key action statements. The 14 recommendations developed emphasize diagnostic accuracy and efficiency, reducing the inappropriate use of vestibular suppressant medications, decreasing the inappropriate use of ancillary testing, and increasing the appropriate therapeutic repositioning maneuvers. An updated guideline is needed due to new clinical trials, new systematic reviews, and the lack of consumer participation in the initial guideline development group.

Chronic kidney disease (CKD), heart failure (HF) and atherosclerotic cardiovascular disease (ASCVD) are pathologies that may remain silent for a long time and thus are largely underdiagnosed in clinical practice. The use of biomarkers may help detect people already suffering from these diseases at an early stage or at increased risk to develop them in a near future. The aim of this article is to discuss the place of the assays of albuminuria, natriuretic peptide (BNP/proBNP) and high-sensitivity troponin as well as lipoprotein(a) to help in the diagnosis and prognosis assessment of individuals at risk of presenting or developing a CKD, HF or ASCVD.

ANCA-associated vasculitis, such as granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA), are rare systemic diseases causing necrotizing inflammation of small blood vessels. Renal involvement is common, leading to acute kidney injury with hematuria and proteinuria. Diagnosis is based on serological tests (PR3-ANCA, MPO-ANCA) and renal histology via biopsy, which helps assess the extent of lesions.

This study aimed to develop a consensus among Latin American experts on vaccination against respiratory diseases in older adults in the region, including influenza, Streptococcus pneumoniae pneumonia, COVID-19, respiratory syncytial virus (RSV), and pertussis.
Methods: A two-round Delphi methodology was employed, involving 35 specialists from various medical fields.

The NCCN Guidelines for Biliary Tract Cancers (BTCs) provide recommendations for the evaluation and comprehensive care of patients with gallbladder cancer, intrahepatic cholangiocarcinoma, and extrahepatic cholangiocarcinoma. The multidisciplinary panel of experts is convened at least once annually to review requests from internal and external entities as well as to evaluate new data on current and emerging therapies. This manuscript focuses on the adjuvant chemotherapy and chemoradiation treatment options for BTCs as well as the systemic treatment recommendations for patients with advanced BTCs.
