Glycine is a major neurotransmitter that activates inhibitory glycine receptors and is a co-agonist for excitatory glutamatergic N-methyl-D-aspartate (NMDA) receptors. Two transporters, GLYT1 and GLYT2, regulate extracellular glycine concentrations within the CNS. Dysregulation of the extracellular glycine has been associated with hyperekplexia and nonketotic hyperglycinemia. Here, we report four individuals from two families who presented at birth with facial dysmorphism, encephalopathy, arthrogryposis, hypotonia progressing to hypertonicity with startle-like clonus, and respiratory failure. Only one individual survived the respiratory failure and was weaned off ventilation but has significant global developmental delay. Mildly elevated cerebrospinal fluid (CSF) glycine and normal serum glycine were observed in two individuals. In both families, we identified truncating mutations in SLC6A9, encoding GLYT1. We demonstrate that pharmacologic or genetic abolishment of GlyT1 activity in mice leads to mildly elevated glycine in the CSF but not in blood. Additionally, previously reported slc6a9-null mice and zebrafish mutants also display phenotypes consistent with the affected individuals we examined. Our data suggest that truncating SLC6A9 mutations lead to a distinct human neurological syndrome hallmarked by mildly elevated CSF glycine and normal serum glycine.

Case History: In 2023, 160/245 (65%) 2-year-old KiwiCross dairy heifers from a seasonally calving Otago herd developed severe granular vulvovaginitis after calving.
Clinical Findings: Affected heifers presented 3-12 days post-calving with tail elevation, vaginal discharge and, in most cases, vulval swelling. Heifers were afebrile although some were inappetent.

This study aimed to identify new sepsis subphenotypes on the basis of coagulation indicator trajectories and comprise clinical characteristics and prognosis.This retrospective study included patients diagnosed with sepsis admitted to the intensive care unit of Peking Union Medical College Hospital from May 2016 to March 2023. Using group-based trajectory models, we classified patients into different subphenotypes on the basis of the dynamic daily changes in coagulation parameters within the first 7 days after sepsis diagnosis.

Recent research has shown that KATP channels in mouse taste bud cells enhance glucose taste signaling by depolarizing the cell when ATP is present. Relatedly, estradiol has been shown to enhance glucose sensing in human pancreatic β cells via closure of KATP channels. Since taste tissue has estradiol receptors, we linked these two observations and tested whether elevated estradiol may also enhance taste sensitivity and liking for glucose in humans.

This is a case of drug-induced liver injury (DILI) in a 75-year-old male patient with a history of metastatic melanoma, who initially presented with a syncopal episode causing a fall. Following stabilization in the emergency department (ED), the patient was found to have bilateral subdural hematomas, and later an MRI showed evidence of metastatic lesions in the brain with hemorrhagic conversion. These findings led to a prolonged inpatient stay in the intensive care unit and eventual development of pneumonitis, which was subsequently treated with hepatotoxic antibiotics despite initial labs showing mildly elevated liver enzymes.

Pruritic urticarial papules and plaques of pregnancy (PUPPP), also known as polymorphic eruption of pregnancy, is a benign inflammatory dermatosis that typically presents in primigravid women during the third trimester of pregnancy. Postpartum onset is rare and often underrecognized. We report a case of a 29-year-old primigravida with a BMI of 38 kg/mwho delivered a healthy 4 kg male infant via emergency cesarean section for fetal distress.
