Relapsing polychondritis (RP) is a rare inflammatory disorder of the cartilagenous structures, and it sometimes involves the central nervous system. Encephalitis associated with RP causes a wide variety of symptoms according to the affected sites. We herein report the first case of 72-year-old right-handed man who developed acute meningoencephalitis associated with RP involving the corpus callous. After immunosuppressive therapy, his symptoms dramatically improved, but difficulty in performing bimanual movements with occasional diagonistic dyspraxia in his right hand remained. Because callosal signs are easily missed, especially in acute settings, it would be useful to know that RP can sometimes cause callosal disconnection syndrome.

Callosal disconnection syndrome is characterized by impaired interhemispheric communication and motor coordination. Although its effects on upper limbs are well documented, reports on lower-limb function are limited, and their pathological mechanisms remain poorly understood. We describe a case involving a right-handed woman in her early 70s who presented with slurred speech and difficulty moving her feet.

The corpus callosum is a large subcortical white matter region in the brain that contains fiber connecting both cerebral hemispheres together; it has a rich blood supply; hence, infarction in this region is rare. There are a few reported cases of corpus callosal infarction, and here we present our patient who had a clinical presentation that was not suggestive of corpus callosal infarction. However, an MRI brain confirmed the location, and the patient was managed following the standard stroke treatment, and vascular risk factors were addressed and managed.

Minimally invasive surgical techniques, such as MR-guided laser interstitial thermal therapy (LITT), have emerged as promising alternatives to open disconnective surgeries in drug-resistant epilepsy (DRE). This review synthesizes current literature on the application of LITT for corpus callosal disconnection and functional hemispheric disconnection. Studies highlight LITT's effectiveness for achieving seizure control and functional outcomes, often with reduced complications compared to traditional open procedures.
