The mayor goal still outstanding into the solid organ transplantation field involves the search of surrogate biomarkers able to predict several clinical events, such as acute rejection (AR) or opportunistic infection. In the present multicenter study, a series of interesting surface antigens with important activator or inhibitory immune functions on cultured peripheral T cells were monitored in liver transplant recipients drawn at baseline and up to one year after transplantation. Sixty-four patients were included in the multicenter study during 3 years. Pre- and post-transplantation surface antigens levels displayed significant differences between AR and non acute rejection (NAR) groups, and also this differential expression was used to construct a risk predictive model based on a composite panel of outcome biomarkers (CD38, CD69, CD95 and CD154). The model was able to stratify these patients at high risk of AR. These preliminary results could provide basic information to improve the immunosuppressive treatment and it might better help to predict AR episodes.

Extracorporeal photopheresis (ECP) is emerging as an apoptotic cell-based therapy that suppresses alloimmunity, promotes donor-specific regulation, and reduces the need for conventional maintenance immunosuppression. ECP therapy is associated with regulatory T-cell proliferation, anti-inflammatory effects, and reduction of anti-HLA antibodies, making ECP a possible alternative or adjunct treatment for preventing and treating transplant rejection. Presently, we have a limited understanding of the mechanisms of ECP action, and clinical evidence for efficacy in kidney transplantation is sparse.

Introduction: We conducted a network meta-analysis (NMA) to compare the efficacy (primarily assessed by low-density lipoprotein cholesterol (LDL-C) reduction and cardiovascular event (CVE) incidence) and safety (total adverse events (AEs), neurocognitive events (NCEs), injection site reactions, infections, and all-cause mortality (ACM)) of different Proprotein convertase subtilisin/kexin type 9 (PCSK9) inhibitors placebo in the general population and solid organ transplant (SOT) recipients.
Materials And Methods: A total of 16 randomized controlled trials (RCTs) involving 79,615 patients were included. Cochrane risk of bias assessment tool evaluated the literature quality.

Liver transplantation remains the definitive treatment for end-stage liver disease, yet rejection of the transplanted organ poses a significant challenge to long-term graft survival. We present a case of a 47-year-old woman who underwent liver transplantation for primary sclerosing cholangitis. Following the procedure, the patient experienced a rare phenomenon of dual rejection, characterized by both acute cellular rejection and antibody-mediated rejection.

Background: Calcineurin inhibitor (CNI) toxicity is a significant cause of graft dysfunction in kidney transplant recipients, yet distinguishing it from acute rejection (AR) and acute tubular necrosis (ATN) remains challenging. This study investigated the use of urinary mRNA biomarkers as a noninvasive tool for identifying CNI toxicity.
Methods: We retrospectively enrolled 110 kidney transplant recipients and classified them into four groups based on pathological findings: stable graft function (n=35), CNI toxicity (n=25), AR (n=30), and ATN (n=20).

In the field of lung transplantation (LTx), the survival of lung transplant recipients (LTRs) is limited by events such as primary graft dysfunction (PGD), infections, and acute rejection (AR), which promote the development of chronic lung allograft dysfunction (CLAD). Extracellular vesicles (EVs), including exosomes and microvesicles, have emerged as key players in LTx because of their roles in immune regulation, inflammation, and antigen presentation. EVs carry immunologically active molecules such as MHC class I/II proteins, cytokines, and lung self-antigens (SAgs), suggesting their involvement in infections and both AR and CLAD.
