Background: Transcranial electrical stimulation muscle-recorded motor evoked potentials (Tc-mMEPs) are used to monitor the spinal cord motor tracts during spinal surgery. There is considerable intra- and interindividual variability in the signals recorded, causing a high incidence of false positive warnings. Inadequate blood pressure is commonly blamed for false positive warnings and is usually managed with fluid and vasopressor therapy.

Background: Alcohol biomarkers including ethyl glucuronide (EtG) and phosphatidylethanol (PEth) are ordered frequently in clinical and forensic settings including solid organ transplantation. PEth provides a long detection window but can be insensitive to light drinking. In contrast, EtG and ethyl sulfate (EtS) can be elevated after light alcohol consumption and might complement PEth testing.

A 56-year old immuno-competent male from a non-endemic region in India presented with progressive weight loss, hoarseness of voice and widespread cutaneous lesions, including leonine facies, genital nodules and diffuse scaling. Magnetic resonance imaging of the neck revealed oedematous thickening of the false vocal cords, epiglottis and aryepiglottic folds, suggesting laryngeal involvement. All routine investigations were normal.
