Article Abstract

Dysembryoplastic neuroepithelial tumors (DNETs) are considered as low-grade tumors commonly associated with intractable seizures. We report a case of an unusual hemispheric DNET in a young child presenting with new-onset focal seizures. The tumor was notable for its atypical neuroimaging features and very rapid malignant transformation into a glioblastoma multiforme in the absence of radiation or chemotherapy, 1-year postdiagnosis. Our case highlights the malignant potential of atypical DNETs in the absence of therapy.

Glioblastoma (GBM) is a lethal brain tumor with limited therapeutic options. Temozolomide (TMZ), a standard-of-care chemotherapeutic agent, exerts its cytotoxicity by alkylating DNA, which triggers a DNA damage response and depletes ATP and NAD. Glioblastoma (GBM), the most prevalent and lethal primary malignancy of the central nervous system, remains refractory to conventional photon radiotherapy due to inherent limitations in dose distribution. Although carbon ion radiotherapy offers distinct advantages, including its characteristic Bragg peak deposition and superior relative biological effectiveness, its clinical application is constrained by high costs and increased toxicity. This study explores the radiobiological interactions underlying a mixed carbon ion-photon irradiation regimen, a promising strategy in advanced particle therapy.

The multifunctional systems presented here introduce an innovative and deeply thought-out approach to the more effective and safer use of temozolomide (TMZ) in treating glioma. The developed hydrogel-based flakes were designed to address the issues of local GBL therapy, bacterial neuroinfections, and the bleeding control needed during tumor resection. The materials obtained comprise TMZ and vancomycin (VANC) loaded into cyclodextrin/polymeric capsules and embedded into gelatin/hyaluronic acid/chitosan-based hydrogel films cross-linked with genipin.
