Cryopreserved monobloc aorto-mitral homograft implantation to treat complex recurrent endocarditis involving the intervalvular fibrous body and both aortic and mitral orifices, as previously described, remains a technically demanding procedure. We report two cases of recurrent destructive aorto-mitral endocarditis treated by a monobloc aorto-mitral homograft implantation with encouraging results.

Surgery for endocarditis of the aorto-mitral continuity can be a challenge in case of extensive tissue destruction. We report two cases of a modified monobloc reconstruction of the aortic and mitral valves and of the aorto-mitral fibrous body. Two valve bioprostheses were sutured to each other and implanted as a composite graft.

The only viable option in prosthetic valve endocarditis with extensive abscess of the aorto-mitral curtain is radical debridement and reconstruction of the intravalvular fibrous body. This report details a simplified intravalvular fibrous body reconstruction technique.
