Crohn's-disease-associated colorectal cancer, where chronic inflammation increases the risk of cancer development, is less common than other types of colorectal cancer. Pathological analyses of Crohn's-disease-associated colorectal cancer are limited. Herein, we present a case of Crohn's disease-associated colorectal cancer, suggesting stepwise carcinogenesis from the chronic inflammatory mucosa.

Introduction: Cefmetazole (CMZ) has gained interest as a carbapenem-sparing alternative to the epidemic of extended-spectrum β-lactamase (ESBL)-producing Enterobacterales (ESBL-E). In this study, we investigated the pharmacokinetics (PK) of CMZ in plasma, peritoneal fluid, peritoneum, and subcutaneous adipose tissue to assess the dosing regimen needed to achieve pharmacodynamic (PD) goals at the target site.

Methods: Patients scheduled for elective lower gastrointestinal surgery were intravenously administered CMZ.

Introduction: Recently, complicated intra-abdominal infections (cIAI) have been caused not only by Escherichia coli, Klebsiella pneumoniae, Enterobacter cloacae, and Pseudomonas aeruginosa, but also by extended-spectrum β-lactamase-producing Enterobacterales members. Ceftolozane-tazobactam (CTLZ-TAZ) is considered to exhibit therapeutic effects against cIAI. Studies on the concentrations of antibiotics in abdominal tissues directly affected by cIAI are limited.

Introduction: Flomoxef is generally used to treat abdominal infections and as antibiotic prophylaxis during lower gastrointestinal surgery. It is reportedly effective against extended-spectrum beta-lactamase (ESBL)-producing Enterobacteriaceae and an increasingly valuable alternative to carbapenems. However, its abdominal pharmacokinetics remain unclear.

Ulcerative colitis (UC), which mainly consists of mucosal lesions, rarely form colovesical or rectovesical fistulas, although few cases of fistula formation associated with comorbidities have been reported. We report a case of UC-associated rectal cancer diagnosed following symptoms associated with rectovesical fistula. A 40-year-old man with a 31-year history of extensive UC presented with difficulty in defecation.

Background: Chronic idiopathic colonic pseudo-obstruction (CICP) is a rare disease, defined as a condition of the chronically damaged colon, without obstruction or stenosis, and a pathological abnormality in the myenteric plexus. To date, there is no effective medication for CICP, and existing medication is not useful, making surgery the only effective treatment. Laparoscopic surgery is useful for reducing surgical trauma and postoperative adhesion.

Article Synopsis
  • - Piperacillin-tazobactam (PIP-TAZ) is often used for treating intraabdominal infections, but its effectiveness at reaching the infection sites in the abdomen was studied through a pharmacokinetic analysis.
  • 10 patients received an intravenous dose of PIP-TAZ before surgery, and samples of blood and peritoneal fluid were taken for analysis of drug concentrations over several hours post-infusion.
  • Results showed a favorable distribution of PIP-TAZ in the peritoneal fluid compared to plasma, with specific dosing regimens identified that can effectively achieve the necessary concentrations to combat infections in the abdominal area.

Background/aim: Cancer of the intestinal tract (small and large intestine) associated with Crohn's disease has a low incidence but can be fatal if it develops. Thus, the key question is how to deal with this type of cancer. The current study surveyed major medical facilities that treat inflammatory bowel disease (IBD) surgically in Japan in order to examine the clinical features of cancer of the intestinal tract associated with Crohn's disease and explore ways to deal with this cancer in the future.
