Pulmonary alveolar microlithiasis (PAM) is a rare idiopathic disease characterized by accumulated calcium and phosphate crystals within the alveoli. Although PAM can be suspected in patients with clinical-radiological dissociation and characteristic imaging findings on chest computed tomography, definitive diagnosis requires a family history of PAM, identification of gene mutations, or lung biopsy to exclude differential diagnoses. We report a case of a 66-year-old female incidentally found to have diffuse pulmonary calcifications.

Rationale: Among 3 forms of mercury, elemental mercury vapor presents the highest threat due to its potential to cause acute pneumonitis. Central airway obstruction often presents with respiratory symptoms identical to those of common respiratory diseases. Diagnosis of central airway obstruction is challenging in clinical practice, especially misdiagnosed as asthma or chronic obstructive pulmonary disease in case of "normal" chest X-ray. Here, we reported five central airway obstruction cases: the first and fourth cases misdiagnosed as asthma, the second masquerading as chronic obstructive pulmonary disease exacerbation, the third diagnosed incorrectly with non-resolving pneumonia, and the fifth misdiagnosed as chronic obstructive pulmonary disease.

  • A study involving 92 patients compared the sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of LBC against CS, confirming final diagnoses via biopsy.
  • Results showed LBC had higher sensitivity (25.7%) and better NPV (19.4%) than CS (sensitivity 15.4%, NPV 15.4%), but the differences weren't statistically significant; the study suggests more research is needed on how fluid volume used in LBC affects its accuracy.
