This practical approach highlights the importance of timely recognition and management of various nephrological emergencies. The featured conditions pose an extraordinary challenge, since their recognition, timely workup and immediate bedside management are often only roughly pictured in guidelines. This article provides case-based insights and highlights pitfalls with regards to current guidelines on the management of acute conditions frequently managed by nephrologists.

We report a case of severe vancomycin-associated anuric acute kidney injury managed with successful drug-removal by hemodialysis (HD) using different types of dialyzers. Medium cut-off (MCO) and high-flux dialyzers were effective in drug removal.

Urgent coronary angiography excluded obstructive coronary artery disease, suggesting the diagnosis of ischemia with non-obstructive coronary arteries. This case illustrates an unusual presentation of cardiovascular involvement in a patient with double-positive ANCA/anti-GBM disease, emphasizing the possible relevance of coronary microvascular dysfunction and the need for close cardiovascular follow-up in this patient population.

Although recommended by the guidelines, divergent positivity rates of Gram-stained effluent microscopy question the value of its usefulness. This study aimed to evaluate if microscopy of cell types serves as an additional and timely diagnostic approach.
