Background: Remimazolam is an ultrashort-acting benzodiazepine that maintains stable hemodynamics during anesthesia. However, few reports have focused on hemodynamic stability and opioid use during cardiac surgery with remimazolam. We hypothesized that administration of remimazolam for induction and maintenance of anesthesia for transcatheter aortic valve implantation would maintain hemodynamics as effectively as conventional anesthetics and allow use of an appropriate dose of opioids.

Repetitive transcranial magnetic stimulation (rTMS) therapy is an effective treatment for treatment-resistant depression (TRD). However, discrepancies between subjective and objective assessments of treatment responses remain underexplored. This study examined discrepancies between clinician-rated and patient-reported scales following acute-phase rTMS therapy, focusing on item-level symptom improvement.

Background: Pediatric asthma remains a significant public health concern in Japan, where adherence to updated treatment guidelines and consistent medication use is inconsistent. This study aimed to evaluate current pediatric asthma management practices and identify areas for improvement to inform future strategies.

Methods: A nationwide, web-based cross-sectional survey was conducted among caregivers of children and adolescents aged 0-19 years diagnosed with asthma and receiving care at medical institutions across Japan.

Objective: Cytokine storm syndrome (CSS), commonly associated with hemophagocytic lymphohistiocytosis (HLH), is a fatal hyperinflammatory syndrome. Differentiating the underlying diseases responsible for CSS is essential for timely therapeutic decisions. This study explored the clinical usefulness of serum cytokine profiling in distinguishing underlying diseases in patients with CSS.

Background: Details of in-hospital stroke are unknown. This study aimed to clarify the incidence rate and risk factors for in-hospital stroke among all inpatients.

Methods: This retrospective single-center study included consecutive patients admitted to our hospital.

Co-administration of multiple intravenous drugs via the Y-site is common in palliative care. Information on the compatibility of opioid analgesics with intravenous drugs commonly used in palliative care is lacking. We aimed to determine the physical compatibility of 4 opioid analgesics with 64 drugs during simulated Y-site administration.

Patients with autoimmune bullous disease have their quality of life (QOL) affected by both the disease and its treatment burden. While QOL assessment is clinically important, it is often hindered by limited time in clinical practice, highlighting the need for accurate and efficient QOL evaluation tools. However, no validated QOL questionnaires are currently available in Japan.

Background: Despite the effectiveness of bariatric surgery, there remains a high rate of attrition from surgical programs, highlighting the need to understand factors that influence presurgical attrition rates.

Objectives: To investigate the association between food addiction (FA) symptomatology and attrition from bariatric surgery.

Setting: Surgical weight-loss program at a single academic medical center.

The objective of this study was to reveal the effectiveness of repetitive transcranial magnetic stimulation (rTMS) for Japanese patients with treatment-resistant depression (TRD) in clinical practice, based on real-world data from a nationwide multicenter observational study in Japan. Clinical data of patients with TRD treated with rTMS (NeuroStar TMS treatment system) under public insurance coverage were retrospectively collected from 21 institutes nationwide between June 2019 and December 2023. Depression severity was assessed by the 17-item Hamilton Depression Rating Scale (HAMD-17).

Article Synopsis
  • Histiocytic necrotizing lymphadenitis (HNL) is a painful inflammatory disease with an unclear cause, and this study focused on the role of interferon (IFN)-α in its development and diagnosis.
  • The study involved 47 HNL patients and 43 others with similar conditions, using various techniques to measure IFN-stimulated genes and serum levels of IFN-α.
  • Results showed higher levels of ISG and serum IFN-α in HNL patients compared to those with malignant lymphoma, suggesting that IFN-α could be a key factor in HNL and a useful marker for its diagnosis and monitoring.

Article Synopsis
  • - The study analyzed the link between the number of guideline-directed medical therapy (GDMT) drugs and mortality in heart failure patients, involving 3,146 participants divided based on their prescribed medications at discharge.
  • - Findings showed that patients with 5 or more GDMT drugs and fewer than 4 non-GDMT (ni-GDMT) drugs had the lowest mortality rates, while those with less GDMT and more ni-GDMT drugs had the highest mortality rates over a three-year period.
  • - The results emphasized that the number of ni-GDMT drugs significantly impacted three-year mortality, suggesting that managing ni-GDMT is essential for improving outcomes in heart failure treatment, while the amount of GDMT alone did not worsen

Introduction: The clinical efficacy of repetitive transcranial magnetic stimulation (rTMS) for treatment-resistant depression (TRD) in Japan has not been adequately investigated. Furthermore, the relationship between stimulation-site pain and the antidepressant effects of rTMS has not been thoroughly examined. Therefore, this study aimed to clarify (1) the real-world efficacy and safety of rTMS for TRD in Japan and (2) the relationship between stimulation-site pain and clinical improvement of depressive symptoms.

Article Synopsis
  • Pimobendan is known to help with the symptoms of heart failure, but there's little evidence showing it improves long-term outcomes.
  • This study evaluated 175 heart failure patients, finding that those with reduced ejection fraction who received guideline-directed medical therapy (GDMT) alongside pimobendan had lower rehospitalization rates compared to those who didn't get GDMT.
  • The results suggest that adding GDMT may be beneficial for specific heart failure patients, but more extensive research is required to confirm these findings.

Article Synopsis
  • * The study investigated producing Mycobacterial DNA-binding protein 1 (MDP1) using two methods, recombinant proteins from E. coli and Mycolicibacterium smegmatis, to test their ability to boost the immune response in BCG-vaccinated individuals.
  • * Results indicated that mMDP1, derived from the avirulent Mycolicibacterium smegmatis, was most effective in stimulating IFN-gamma production when combined with a novel CpG DNA called G9.1

Red blood cell (RBC) transfusion therapy is often performed in patients with acute heart failure (AHF) and anemia; however, its impact on subsequent cardiovascular events is unclear. We examined whether RBC transfusion influences major adverse cardiovascular events (MACE) after discharge in patients with AHF and anemia. We classified patients with AHF and anemia (nadir hemoglobin level < 10 g/dL) according to whether they received RBC transfusion during hospitalization.
