A Case of Intraocular Pressure Elevation Secondary to Pupillary Occlusion.

We report a case of acute intraocular pressure (IOP) elevation caused by pupillary occlusion in a Japanese man in his 90s. The patient presented with a left frontal headache and vomiting, and ophthalmologic evaluation revealed marked IOP elevation, a shallow anterior chamber, and pupillary membrane formation obscuring the lens. While anterior segment optical coherence tomography (AS-OCT) was unable to confirm the lens position, ultrasound biomicroscopy (UBM) clearly demonstrated the absence of lens displacement or intraocular mass.

Comparison of Standalone Tanito Microhook Trabeculotomy Between Unilateral and Bilateral Incision Groups.

Trabeculotomy using the Tanito microhook (TMH) is a minimally invasive glaucoma surgery (MIGS) technique that effectively reduces intraocular pressure (IOP). The optimal incision extent for standalone TMH remains unclear. This study aimed to compare the surgical efficacy and safety of unilateral (120-degree) and bilateral (240-degree) incisions in standalone TMH for primary open-angle glaucoma or late-onset juvenile open-angle glaucoma in patients without a history of intraocular surgery.

A Case of Long-Term Undiagnosed Cyclodialysis Cleft Following Tanito Microhook Trabeculotomy.

Persistent hypotony following Tanito microhook trabeculotomy (TMH) is rare but may occur due to the development of cyclodialysis clefts. We report a case of a Japanese man in his 40s who developed persistent hypotony and hypotony maculopathy after TMH in the left eye. Fourteen months after the surgery, the patient was referred to our institution due to prolonged hypotony that remained undiagnosed and untreated despite evaluations with gonioscopy and anterior segment optical coherence tomography (AS-OCT) at the referring clinic.

Tube Insertion of Ahmed Glaucoma Valve Using a Micro-incision Scleral Tunnel Technique.

We report three cases demonstrating the efficacy and versatility of the micro-incision scleral tunnel (MIST) technique, a novel method for Ahmed glaucoma valve (AGV) tube insertion. MIST is characterized by its small incision, sutureless approach, anterior-to-posterior tunnel creation, and allograft-free design. The technique involves creating a scleral tunnel using a 1-mm crescent knife (Bleb Knife II), allowing for secure tube placement into the anterior chamber, ciliary sulcus, or vitreous cavity.
