Systemic amyloidosis is a complex disorder, making early and accurate diagnosis challenging. The most common types are associated with misfolded transthyretin or immunoglobulin light chains, where cardiac and renal amyloidosis portend the worst prognosis. Peptide p5+14 can bind all types of amyloid via multivalent electrostatic interactions.

Costs for UpToDate, the library's primary point of care clinical tool, had increased to half of the acquisitions budget but without user affiliation data to facilitate cost sharing. A working group led by librarians was formed to review alternatives. Surveys indicated users preferred UpToDate and renewal was recommended by the working group, with costs being shared between the academic unit and the hospital.

Background: Following transcatheter aortic valve implantation (TAVI), transvalvular mean gradient is known to increase from immediate to 24 h post-procedure. While anesthesia, rapid-pacing, and volume status are blamed, the true etiology is unclear. To our knowledge, no prior studies have evaluated the effects of mitral regurgitation (MR) on the rise in post-TAVI transvalvular mean gradient.

Introduction: Robotic retromuscular ventral hernia repair (r-RVHR) may benefit patients by converting an open surgery (o-RVHR) to a minimally invasive approach. Current comparative trials are limited by small patient cohorts and exploratory outcomes. This study compares short- and long-term outcomes of robotic versus open retromuscular ventral hernia repairs using a nationwide registry.

Objective: Patient-centered care is a core value of both Family Medicine and Obstetrics & Gynecology. We sought to know if patients prefer being offered a mirror to see their cervix and external genitalia during asymptomatic speculum exams (Mirror Pelvic Exam, MPE). Additionally, we explored clinicians' (medical assistants, residents, and faculty) opinions about offering patients a mirror during exams.

Background: A single-institution study performed by our authors demonstrated that machine learning (ML) utilizing preoperative anthropometric variables was an accurate alternative to the Schnur Scale in predicting resection weights during reduction mammaplasty (RM). We sought to evaluate ML and regression modeling in a heterogenous multi-institutional population for predicting RM resection weights with improved accuracy and generalizability.

Methods: A multi-institutional retrospective study was performed including 635 patients from three institutions who underwent RM for macromastia between 2017 and 2022.

The authors present a novel approach using a vascularized double-barrel fibula flap combined with an intramedullary nail (IMN) to reconstruct critical-size segmental tibial defects. Current methods, such as bone transport or induced membrane techniques, require multiple procedures and are associated with high complication rates and delayed load bearing. We present the case of a 44-year-old man with nonunion and chronic osteomyelitis of the left tibia, who underwent reconstruction with a contralateral double strut free fibula flap, placement of IMN, and cancellous bone grafting.

Prenatal Diagnosis Post-Dobbs in an Abortion-Banned State: A Retrospective Cohort Study.

J Womens Health (Larchmt)

June 2025

Department of Obstetrics and Gynecology, division of Maternal Fetal-Medicine, High Risk Obstetrical Consultants, University of Tennessee Graduate School of Medicine, Knoxville, TN, USA.

After the Supreme Court decision in overturned , a complete abortion ban went into effect in Tennessee. Abortion restrictions may influence decisions regarding prenatal genetic screening and diagnostic testing; however, the exact impacts are unknown. We aimed to describe any differences in the method, timing, or indications for diagnostic genetic testing between patients presenting to a single regional referral site before and after the decision.

Introduction: For primary bilateral inguinal hernias, international guidelines favor a laparoscopic posterior mesh repair due to relatively lower risk of acute and chronic pain, faster recovery, and favorable biomechanical properties compared to open anterior approaches (Lichtenstein, plug and patch, etc.). However, studies comparing open mesh-based bilateral inguinal hernia repairs to bilateral laparoscopic and robotic mesh-based approaches are limited.

: After transcatheter aortic valve implantation (TAVI), transvalvular gradients increase immediately following the procedure up to 24 h afterward. While factors such as anesthesia type and fluid status have been suggested as potential contributors, the underlying cause remains unclear. With advancements in TAVI techniques, there has been a shift in anesthesia protocols from general anesthesia (GA) to monitored anesthesia care (MAC).

Background: Over 47,000 human papillomavirus (HPV) related cancers are newly diagnosed every year in the United States, yet HPV vaccination rates are below desirable levels. Young adulthood is critical for catch-up HPV vaccination. Previous research on HPV vaccine-seeking behavior among young adults is limited, particularly in Tennessee.

Purpose: Postoperative opioid-induced respiratory depression (POIRD) is a preventable perioperative cause of morbidity and mortality. A validated POIRD risk stratification tool could reduce these complications. 3 pre-existing validated opioid tools; and specific risk factors identified from these tools; were examined in this retrospective case-control study to determine if they could assess POIRD risk in patients discharged to hospital floors from the Post-Anesthesia Care Unit (PACU).

This column explores gender trends over the past decade in medical and health sciences libraries through data reported from the Association of Academic Health Sciences Libraries (AAHSL) and Association of Research Libraries' (ARL) salary surveys of their member institutions. Trends are related to gender composition and percentage of men in libraries are examined. Specifically, the column probes whether the gender composition in academic medical libraries has varied, whether the percentage of men in reporting libraries is increasing, and whether men are overrepresented in administrative positions.

Job postings for medical and health sciences librarians provide valuable data for those seeking a better understanding of the evolving field of librarianship. Our data indicate a decrease in the number of postings from 2022 to 2024, a modest increase in the percentage of postings advertising remote/hybrid work, an increase in the average minimum posted salary, and a majority of postings focused on one or more public service components of library services. Utilizing this data provides a more complete picture of a profession in transition.

Cancer therapy-induced oral mucositis is a frequent major oncological problem, secondary to cytotoxicity of chemo-radiation treatment. Oral mucositis commonly occurs 7-10 days after initiation of therapy; it is a dose-limiting side effect causing significant pain, eating difficulty, need for parenteral nutrition and a rise of infections. The pathobiology derives from complex interactions between the epithelial component, inflammation, and the oral microbiome.
